Abstract

โรคมะเร็งเป็นโรคเรื้อรังที่รักษาให้หายได้ยาก การรักษาด้วยยาเคมีบำบัดเป็นอีกแนวทางการรักษาหนึ่ง โดยภาวะ แทรกซ้อนของยาเคมีบำบัดมีความรุนแรงมากถึงขั้นเสียชีวิตได้ หากมีพฤติกรรมการดูแลตนเองที่ไม่ดี การวิจัยครั้งนี้ มีวัตถุประสงค์ เพื่อศึกษาปัจจัยที่มีความสัมพันธ์กับพฤติกรรมการดูแลสุขภาพตนเองของผู้ป่วยโรคมะเร็งที่ได้รับการรักษาด้วยยาเคมีบำบัด เป็นวิจัยเชิงวิเคราะห์แบบภาคตัดขวาง ประชากรที่ศึกษาเป็นผู้ป่วยโรคมะเร็งที่มารับบริการหน่วยมะเร็งวิทยา กองอายุรกรรม (Medical oncology Unit) โรงพยาบาลพระมงกุฎเกล้า จำนวน 280 คน จำแนกเป็น 2 กลุ่ม คือ กลุ่มพฤติกรรมการดูแลสุขภาพ ตนเองดีและกลุ่มพฤติกรรมการดูแลสุขภาพตนเองไม่ดี เครื่องมือที่ใช้ในการวิจัยคือแบบทดสอบความรู้และแบบสอบถาม วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา ค่าการทดสอบไคสแควร์ (Chi-square) และวิเคราะห์การถดถอยพหุลอจิสติก (Multiple logistic regression)

ผลการวิจัยพบว่า ประชากรที่ศึกษาเป็นผู้ที่มีพฤติกรรมการดูแลสุขภาพตนเองที่ไม่ดี คิดเป็นร้อยละ 40.4 มีพฤติกรรม การดูแลสุขภาพตนเองที่ดี คิดเป็นร้อยละ 59.6 ปัจจัยที่มีความสัมพันธ์กับพฤติกรรมการดูแลสุขภาพตนเองของผู้ป่วยโรคมะเร็ง อย่างมีนัยสำคัญทางสถิติ ประกอบด้วย 1) ปัจจัยส่งเสริมให้มีพฤติกรรมการดูแลสุขภาพตนเองได้ดี ได้แก่ อายุระหว่าง 61 - 70 ปี (ORAdj = 0.32, 95%CI: 0.11- 0.91, p = 0.032) และการรับรู้ความรุนแรงของภาวะแทรกซ้อนระดับน้อย (ORAdj = 0.49, 95%CI: 0.24-1.00, p = 0.049) และ 2) ปัจจัยเสี่ยงให้มีพฤติกรรมการดูแลสุขภาพตนเองได้ไม่ดี ได้แก่ การรับรู้อุปสรรคของการดูแลสุขภาพ ตนเองระดับน้อย (ORAdj = 5.06, 95%CI: 2.31-11.12, p = 0.000) และการได้รับการสนับสนุนจากเจ้าหน้าที่ทางการแพทย์ และสาธารณสุขระดับน้อย (ORAdj = 3.02, 95%CI: 1.46-6.25, p = 0.003)

 

Cancers is the chronic disease and difficult to cure treatment. Chemotherapy is another one for health it. The side effect of chemotherapy may be had dead if you cannot have good Self - health care behaviors. This research is Cross-sectional analytic studies. This research aimed to study the factors which were associated with self - health care behaviors. The studied population was 280 cancer patients who attended the Medical Oncology Unit, Phramongkutklao Hospital. The subjects were devided into two groups: good self - health care behaviors and poor self - health care behaviors. The instruments of research were cognitive test and questionnaire. The data were analyzed by descriptive statistics, chi-square test, and multiple logistic regressions.

The findings of this study revealed that there were 40.4 percent of poor self - health care behaviors, and 59.6 percent of good self - health care behaviors. The factors which significantly statistically associated with self- health care behaviors of cancer patients who had received chemotherapy were 1) good self - health care behaviors promotion factors such as age 61-70 years old (ORAdj = 0.32, 95%CI: 0.11- 0.91, p = 0.032), and low perceived severity of complication chemotherapy (ORAdj = 0.49, 95%CI: 0.24-1.00, p = 0.049), and 2) poor self - health care behaviors risk factors such as high perceived barriers of self - health care (ORAdj = 5.06, 95%CI: 2.31-11.12, p = 0.000), and low supporting from medicine and public health (ORAdj = 3.02, 95%CI: 1.46-6.25, p = 0.003).
