การให้บริการสุขภาพเชิงรุกของพยาบาลที่ปฏิบัติงานในโรงพยาบาลชุมชน พื้นที่ติดชายแดนภาคตะวันตก Proactive Health Care Service of Nursesworking in a Community Hospital at west Border Region

Abstract

การวิจัยครั้งนี้มีวัตถุประสงค์เพื่อบรรยายการให้บริการสุขภาพเชิงรุกของพยาบาลที่ปฏิบัติงานในโรงพยาบาลชุมชนพื้นที่ติดชายแดนภาคตะวันตก โดยใช้ระเบียบวิธีวิจัยแบบปรากฏการณ์วิทยาการตีความตามแนวคิดของ Heidegger คัดเลือกผู้ให้ข้อมูลแบบเฉพาะเจาะจง โดยผู้ให้ข้อมูล คือ พยาบาลวิชาชีพที่ปฏิบัติงานเชิงรุกในโรงพยาบาลชุมชน พื้นที่ติดชายแดนภาคตะวันตก มีความยินดีในเข้าร่วมการวิจัยครั้งนี้ จำนวน 15 ราย เก็บรวบรวมข้อมูลโดยการสัมภาษณ์แบบเจาะลึกและการบันทึกเทป ร่วมกับการบันทึกภาคสนาม นำข้อมูลที่ได้มาถอดความแบบคำต่อคำ และวิเคราะห์ข้อมูลโดยวิธีการตีความของ van Manen (1990) ผลการศึกษา มีดังนี 1) ร่วมวางแผนการเดินทางกับทุกภาคส่วนงาน 2) เดินทางเข้าหมู่บ้าน ผ่านเส้นทางอันตราย ต้องใช้ผู้ชำนาญทาง 3) เข้าถึงพื้นที่ เริ่มงานทันที ตามหน้าที่ที่แบ่งกัน 4) ออกจากพื้นที่หนึ่ง ไปให้ถึงอีกพื้นที่ก่อนพลบค่ำ 5) ดำเนินการตรวจรักษาชาวบ้านมากหน้าหลายตา มารอรับบริการและ 6) เสร็จงานออกจากพื้นที่ บางครั้งมีปัญหา กลับโรงพยาบาลล่าช้า

The purpose of thisstudy was to describe proactive health care service of nurse in a community hospital at west border region. Hermeneutic phenomenology of Heidegger was employed as research methodology. Fifteen professional nurses working in a community hospital at west border region were willing to participate in the study. Data were collected by using in-depth interviews with audio-recorded and field observations. Data were transcribed verbatim and analyzed by thematic analysis proposed by van Manen (1990). The results of the research revealed as follows: 1) Joining with team to set a plan of proactive health care service 2) Using a good navigator and driver brought a team to village 3) Reaching to the village, anyone just start his/her work, 4) Departing from the first area to the second area before dusk, 5) Proceeding the treatment to various people that are waiting for the service, and 6) Leaving from the last area may have the problems on the way back to the hospital.
