Abstract

บทคัดย่อ: วัตถุประสงค์ของการวิจัย: ศึกษาอำนาจการทำนายของสภาพร่างกายก่อนการผ่าตัด  ดัชนีมวลกาย  สภาพผู้ป่วยระหว่างผ่าตัดกับการเกิดกลุ่มอาการตอบสนองการอักเสบทั่วร่างกายของผู้ป่วยที่ได้รับการผ่าตัดช่องท้องในระยะ 24 ชั่วโมงแรก

การออกแบบงานวิจัย: การวิจัยเชิงการทำนาย

การดำเนินการวิจัย: กลุ่มตัวอย่างเป็นผู้ป่วยโรคระบบทางเดินอาหาร ตับและทางเดินน้ำดี 90 รายที่มีการนัดหมายล่วงหน้า  เครื่องมือที่ใช้ในการวิจัย  ประกอบด้วย  แบบประเมินสภาพร่างกายก่อนการผ่าตัด แบบบันทึกดัชนีมวลกาย แบบประเมินสภาพผู้ป่วยระหว่างผ่าตัด และแบบประเมินกลุ่มอาการตอบสนองการอักเสบทั่วร่างกาย  วิเคราะห์ข้อมูลโดยใช้สถิติวิเคราะห์ถดถอยโลจิสติค

ผลการวิจัย: กลุ่มตัวอย่างส่วนใหญ่เป็นเพศชายร้อยละ 52 อายุเฉลี่ย 62.36 ปี (SD=15.05) เกิดกลุ่มอาการตอบสนองการอักเสบทั่วร่างกายในระยะ 24 ชั่วโมงแรกร้อยละ 70 โดยสภาพร่างกายก่อนการผ่าตัดและสภาพผู้ป่วยระหว่างผ่าตัดสามารถทำนายการเกิดกลุ่มอาการตอบสนองการอักเสบทั่วร่างกายได้อย่างมีนัยสำคัญทางสถิติตามลำดับ ( p =.010 และ .000)

ข้อเสนอแนะ: ผู้ป่วยผ่าตัดช่องท้องทุกรายควรมีการเตรียมความพร้อมสภาพร่างกายก่อนเข้ารับการผ่าตัด และควรได้รับการประเมินสภาพผู้ป่วยระหว่างผ่าตัด เพื่อเฝ้าระวังอย่างต่อเนื่อง

Abstract: Objective: To examine abdominal surgery patients’ pre-operative physical conditions and body mass indices as predictive factors for systemic inflammatory response syndromes during the first 24 post-operative hours. Design: Predictive research. Implementation:The sample consisted of 90 gastro-intestinal, hepatic and bile tract disorder patients, all of whom had surgery appointments. The research instruments consisted of (i) a pre-operative physical condition assessment (ASA classification); (ii) a body mass index scale (BMI scale); (iii) a surgical Apgar score; and (iv) a systemic inflammatory response syndrome scale (SIRS scale). The data were analysed using logistic regressive analysis statistics. Results: The significant findings of the study are as follows. More than half (52%) of the patients studied were male, with an average age of 62.36 years (SD=15.05). Systemic inflammatory response syndromes were detected in about 70% of the patients during the first 24 post-operative hours. The patients’ pre-operative physical conditions and surgical Apgar scores displayed statistically significant predictive power, at p =.010 and p = .000, respectively. Recommendations: It is recommended that a pre-operative physical readiness programme be administered to every abdominal surgery patient. In addition, the patient’s during-operation condition should be continuously assessed and monitored.
