Abstract

บทคัดย่อ: วัตถุประสงค์การวิจัย: เพื่อศึกษาทรรศนะของครอบครัวผู้ป่วยมะเร็งระยะสุดท้ายเกี่ยว กับความปรารถนาในบั้นปลายของชีวิต ความคิด ความเชื่อ และความต้องการด้านจิตวิญญาณ สภาวะทางอารมณ์และความไม่สุขสบายทางร่างกาย รวมทั้งความต้องการการช่วยเหลือ

การออกแบบวิจัย: วิจัยเชิงบรรยาย

วิธีดำเนินการวิจัย: กลุ่มตัวอย่าง คือ ผู้ดูแลที่เป็นตัวแทนครอบครัวซึ่งดูแลผู้ป่วยมะเร็งระยะ สุดท้ายจำนวน 100 ราย รวบรวมข้อมูลโดยใช้ 1) แบบสอบถามข้อมูลทั่วไป 2) แบบประเมินทรรศนะ ของครอบครัว และ 3) แบบประเมินความต้องการการช่วยเหลือของครอบครัว วิเคราะห์ข้อมูลด้วย สถิติบรรยาย

ผลการวิจัย: 1) ความปรารถนาในบั้นปลายชีวิตของผู้ป่วยที่มีคะแนนเฉลี่ยสูงสุด คือ ประสงค์ ใช้ชีวิตในระยะสุดท้ายและเสียชีวิตที่บ้าน ( = 3.38 , SD = 1.15) 2) ความคิด ความเชื่อ และ ความต้องการด้านจิตวิญญาณที่มีคะแนนเฉลี่ยสูงสุด คือ ให้ความสำคัญกับบุคคลที่รักและผูกพัน ( = 4.88, SD = 0.43) และ 3) สภาวะทางอารมณ์ พบว่า ความวิตกกังวลมีคะแนนเฉลี่ยสูงสุด ( = 4.70, SD = 2.65) และความไม่สุขสบายทางร่างกายที่มีความรุนแรงมาก 3 อันดับแรก ได้แก่ 1) เหนื่อยล้า/อ่อนเพลีย ( = 7.44, SD = 1.99) 2) ปวด ( = 6.53, SD = 3.16) และ 3) ง่วงซึม/สะลึมสะลือ ( = 6.23, SD = 3.16) ส่วนความต้องการการช่วยเหลือของครอบครัว โดยรวมอยู่ในระดับปานกลาง ( = 1.92, SD = 0.78) โดยความต้องการการช่วยเหลือในการ ดูแลผู้ป่วย สูงกว่าความต้องการการช่วยเหลือสำหรับผู้ดูแลเอง

ข้อเสนอแนะ: ผลการศึกษานี้สามารถนำไปเป็นแนวทางในการช่วยเหลือดูแลผู้ป่วยมะเร็ง ระยะสุดท้ายและครอบครัวอย่างเหมาะสมต่อไป

Abstract: Objective: To study the views of terminal cancer patients’ family caregivers concerning the patients’ wishes, thoughts, beliefs, spiritual needs, emotional condition, physical discomfort, and needs for assistance at the end-stage of their lives.

Design: Descriptive research.

Implementation: The subjects were 100 people who represented the families of terminal cancer patients. The data were collected through (1) a personal information questionnaire; (2) a family caregiver’s view questionnaire; and (3) a questionnaire for family’s needs for assistance, and were analysed using descriptive statistics.

Results: The subjects’ (caregivers’) views are as follows. First, spending the last stage of life and dying at home was the patients’ most important wish ( = 3.38, SD = 1.15). Second, in terms of thoughts, beliefs, and spiritual needs, the subjects on average rated attention to loved ones and intimates as being the most significant ( = 4.88, SD = 0.43). Third, anxiety was identified as the most aggravating factor affecting the patients’ emotional condition ( = 4.70, SD = 2.65). Fourth, the three most serious forms of physical discomfort identified by the subjects as affecting the patients were fatigue ( = 7.44, SD = 1.99), pain ( = 6.53, SD = 3.16), and drowsiness ( = 6.23, SD = 3.16). In general, the subjects viewed that family caregivers’ help was moderately necessary ( = 1.92, SD = 0.78), and that the patients were in greater need of healthcare than their family caregivers were.

Recommendations: The findings of this study may be developed into a case-specific care programme for terminal cancer patients and their family caregivers.
