Abstract

บทคัดย่อ : วัตถุประสงค์ของการวิจัย : เพื่อศึกษาผลของโปรแกรมการใช้ดนตรีบำบัดร่วมกับ การสนับสนุนทางสังคมต่อภาวะซึมเศร้าในผู้สูงอายุโรคพาร์กินสัน

การออกแบบวิจัย : การวิจัยกึ่งทดลอง

การดำเนินการวิจัย : กลุ่มตัวอย่างคือ ผู้สูงอายุที่ป่วยด้วยโรคพาร์กินสันทั้งเพศชาย และเพศหญิง ที่เข้ารับบริการในคลินิกพาร์กินสัน หอผู้ป่วยนอก สถาบันประสาทวิทยา จำนวน 40 คน แบ่งเป็นกลุ่มควบคุมและกลุ่มทดลอง กลุ่มละ 20 คน โปรแกรมการทดลองใช้ดนตรี เพื่อการบำบัดร่วมกับการสนับสนุนทางสังคม ประกอบด้วย 3 กิจกรรม คือ 1) กิจกรรม กลุ่มรอบรู้สู้พาร์กินสัน 2) กิจกรรมกลุ่มคีตะสัมพันธ์ จัดในโรงพยาบาลจำนวน 2 ครั้ง 3) กิจกรรมรายบุคคลคีตะเจริญใจ จัดต่อเนื่องที่บ้านอย่างน้อยวันละครั้ง ไม่ต่ำกว่า 4 ครั้งต่อ สัปดาห์ โปรแกรมนี้ใช้ระยะเวลา 4 สัปดาห์ เครื่องมือที่ใช้เก็บรวบรวมข้อมูลได้แก่ แบบสอบถาม ข้อมูลส่วนบุคคล แบบประเมินภาวะซึมเศร้า CES-D ฉบับภาษาไทยของ Radloff วิเคราะห์ ข้อมูลโดยใช้ค่าเฉลี่ย ร้อยละ ส่วนเบี่ยงเบนมาตรฐาน สถิติทดสอบค่าที (t-test)

ผลการวิจัย : พบว่า ภาวะซึมเศร้าของผู้สูงอายุโรคพาร์กินสัน หลังจากได้รับโปรแกรม การใช้ดนตรีเพื่อการบำบัดร่วมกับการสนับสนุนทางสังคมมีค่าคะแนนเฉลี่ยต่ำกว่าก่อนได้รับ โปรแกรมฯ อย่างมีนัยสำคัญทางสถิติ (p<.05) และภาวะซึมเศร้าของผู้สูงอายุโรคพาร์กิน สันกลุ่มที่ได้รับโปรแกรมการใช้ดนตรีเพื่อการบำบัดร่วมกับการสนับสนุนทางสังคมมีค่า คะแนนเฉลี่ยต่ำกว่ากลุ่มที่ได้รับการพยาบาลตามปกติ อย่างมีนัยสำคัญทางสถิติ (p<.05)

ข้อเสนอแนะ : การนำโปรแกรมนี้ไปใช้ควรมีการปรับรายละเอียดของกิจกรรมให้เหมาะสม กับบริบทของผู้ป่วยหรือกลุ่มเป้าหมายในแต่ละพื้นที่ โดยยึดหลักปฏิบัติและมาตรฐานของ กิจกรรมเช่นเดียวกัน ครอบคลุมทั้งดนตรีบำบัดและการสนับสนุนทางสังคม

Abstract : Objective: To study the impact that co-application of music therapy and social
support could have on depression in elderly patients suffering from Parkinson’s disease.
Design: Experimental research.
Implementation: This study was conducted on a sample of 40 elderly patients
who suffered from Parkinson’s disease and were being treated at the Parkinson Clinic
of Prasat Neurological Institute’s Out-Patient Department. The participants were
equally divided into a control group and an experimental group. Whilst the control group
was given standard care, the experimental group members received, in addition to
standard care, concurrently implemented music therapy and social support, a 4-week
model comprising 3 activities: (1) Parkinson’s disease education; (2) 2 music sessions
in the hospital; and (3) the ‘Joy through Music’ activity held at least once a day and at
least 4 times per week. The data collection instruments were (1) a personal information
form and (2) a Thai version of Radloff’s CES-D depression evaluation. The data were
analysed in terms of mean, percentage, standard deviation and t-test statistics.
Results: After the co-application of the music-and-social-support model, the
experimental group members displayed a significantly decreased level of depression
(p < .05). Similarly, the experimental group members’ degree of depression was
significantly lower than that of their control group counterparts (p < .05).
Recommendations: This model can be customised for specific groups of patients,
depending on their hospitals’ contexts. Whilst the model’s basic procedure and standard
are maintained, details may be adjusted to ensure adequate coverage of both music therapy
and social support.
