Results of Self-management Program on Health Status and Quality of Life in Post Percutaneous Coronary Intervention Patients

Objective: To study the results of self-management program on health status and quality of life in patients with coronary artery disease (CAD) post percutaneous coronary intervention (PCI).

Methods: This was a pre- and post-tested quasi-experiment single-group research. Subjects were 30 patients with CAD who had PCI at the Faculty of Medicine Vajira Hospital. Subjects were exposed to the self-management program which focused on an improvement of self-efficacy, empowerment in lifestyle modification, and setting a goal for self-management. This was conducted by providing instruction, information, consultation, and individual self evaluation. Subjects also received manual of CAD self-care, and were visited once prior to a hospital discharge. Follow-up visits were achieved by two phone calls at one and three weeks after being discharged. Health status and quality of life were evaluated on day 1 post PCI before receiving self-management program and at 1 month later. Health status was evaluated by visual analog score, while quality of life was measured by SF-36 (short form 36 health survey), which composed of 8 components. The pre- and post-enrollment health status, quality of life scores in total and according to each component were compared.

Results: Slightly more than half of the subjects (53.3%) were male. Mean age of all subjects was 60.9 ฑ 14.7 years. The mean scores of health status and total quality of life of the subjects after an enrollment to the self-management program were significantly higher than the pre-enrollment, 8.1 ฑ 1.8 and 93.4 ฑ 5.5 versus 2.5 ฑ 2.1 and 4.2 ฑ 7.1, respectively. The mean scores of quality of life in all 8 components after enrollment to a self-management program were significantly higher compared to those before the enrollment as the followings: 84.7 ฑ 16.4 vs 10.2 ฑ 13.9 for physical function; 95.8 ฑ 11.5 vs 0 for restricted role due to physical health problems; 93.5 ฑ 6.6 vs 6.9 ฑ 19.9 for pain; 93.3 ฑ 9.7 vs 4.9 ฑ 14.1 for social function, 96.0 ฑ 4.8 vs 4.8 ฑ 2.6 for general mental health, 100.0 ฑ 0.0 vs 0 for restricted role due to emotional problems, 93.0 ฑ 7.8 vs 5.0 ฑ 9.9 for vitality; and 87.0 ฑ 8.9 vs 1.6 ฑ 4.5 for general health perception.
