         การศึกษาครั้งนี้เป็นการวิจัยเชิงสำรวจ แบบตัดขวาง เพื่อศึกษาความแตกฉานด้านสุขภาพในกลุ่มเสี่ยงโรคความดันโลหิตสูงและศึกษาความสัมพันธ์ระหว่างความแตกฉานด้านสุขภาพกับการปฏิบัติตัวเพื่อป้องกันโรคของกลุ่มเสี่ยงโรคความดันโลหิตสูง  และปัจจัยต่างๆ โดยการสุ่มตัวอย่างจากกลุ่มเสี่ยงโรคความดันโลหิตสูง ที่ได้รับการตรวจคัดกรองและขึ้นทะเบียนในปีงบประมาณ 2554 ของโรงพยาบาลส่งเสริมสุขภาพตำบลบ้านหนองหอย จังหวัดสระแก้ว จำนวน 200 ราย เก็บรวบรวมข้อมูลโดยใช้แบบสอบถามข้อมูลทั่วไป     แบบประเมินความแตกฉานด้านสุขภาพสำหรับกลุ่มเสี่ยงความดันโลหิตสูง (Reading comprehension) และแบบประเมินทักษะที่จำเป็นในการเรียนรู้เพื่อการปรับเปลี่ยนพฤติกรรมการป้องกันโรคความดันโลหิตสูง วิเคราะห์ข้อมูลด้วยสถิติ ร้อยละ และสถิติ     ไค-สแควร์ (Chi-Square test)

         ผลการศึกษาพบว่า กลุ่มเสี่ยงโรคความดันโลหิตสูงมีความแตกฉานด้านสุขภาพในระดับก้ำกึ่ง ระดับไม่เพียงพอ  และระดับเพียงพอร้อยละ 51.5, 37.5 และ11 ตามลำดับ ความแตกฉานด้านสุขภาพที่ต้องปรับปรุง ได้แก่  ความสามารถในการอ่านและจดจำประเด็นหลักที่สำคัญเรื่องโรคความดันโลหิตสูง (ร้อยละ  78.5)  รองลงมาคือความสามารถในการสืบค้นข้อมูลที่ถูกต้องและทันสมัย ( ร้อยละ 73.5) ความหมายของดัชนีมวลกาย (ร้อยละ 66) อาหารที่มีส่วนประกอบของโซเดียมสูง (ร้อยละ 65.5)  ภาวะแทรกซ้อนที่เกิดจากโรคความดันโลหิตสูง (ร้อยละ 65)  สำหรับการปฏิบัติตัวในการป้องกันโรคพบว่าอยู่ในเกณฑ์พอใช้ ร้อยละ 49.5 และเกณฑ์ต้องปรับปรุงร้อยละ 50.5  เมื่อพิจารณาตามการปฏิบัติตัวพบว่า มีการเติมเครื่องปรุงรส เช่น เกลือป่น น้ำปลา  ซีอิ๊วขาว ผงชูรสพบมากถึงร้อยละ 72.5 ความแตกฉานด้านสุขภาพ มีความสัมพันธ์กับปัจจัยด้าน ระดับการศึกษา  สถานภาพอย่างมีนัยสำคัญทางสถิติ (p <0.05) และมีความสัมพันธ์กับการปฏิบัติตัวเพื่อป้องกันโรคความดันโลหิตสูงอย่างมีนัยสำคัญทางสถิติ (p <0.05)  จากผลจากการศึกษานี้ หน่วยบริการปฐมภูมิและหน่วยงานที่เกี่ยวข้องควรการจัดบริการสร้างเสริมพฤติกรรมการดูแลตนเองเพื่อป้องกันการเกิดโรคความดันโลหิตให้แก่กลุ่มเสี่ยงได้อย่างเหมาะสมกับระดับความแตกฉานด้านสุขภาพ

Abstract

      This cross-sectional  survey  aimed to study the health literacy and  its factors among persons who are at risks for hypertention.  The participants were 200 persons  from  a  group at risk of  hypertension  who were screened and registered in the fiscal year 2011  at the Health Promoting Hospital of Ban Nonghoi Sub-District, Sa Kaeo Province. Information  was collected through questionnaires created by the researcher.   The data were analyzed by frequency, percentage  and Chi-Square test.

      The results showed that the hypertention risk group has health literacy at the marginal, inadequate, or adequate levels at 51.5, 37.5, and 11 percent. The information about hypertension that most of people have is wrong or they do not fully understand and they are unable to read and memorize the main points is important for literacy about hypertension (78.5 %), followed by the ability to search for information that is accurate and up to date (73.5 %), understanding the meaning of body mass index (66 %), knowledge about high- sodium foods consumption (65.5 %), and knowledge of complications caused by hypertension (65 %).  Practices to prevent disease to be at a fair level (49.5 %) or  to be at a level which requires improvement (50.5 %).  When considering behavior, it was found that the addition of condiments such as salt, fish- sauce, soy sauce and monosodium glutamate in food more found to be as high as 72.5 %.  The factors that affect health literacy are education level and marital (p <0.05). Health literacy has a statistically significant relationship with practices to prevent  hypertension (p <0.05). The results of this research will benefit Primary Care Units (PCUs) and related agencies in the service delivery of promoting self-care behaviors to appropriately  prevent  hypertension in groups at risk.
