การวิจัยเชิงปฏิบัติการนี้เพื่อ พัฒนาศักยภาพในการดูแลต่อเนื่องผู้ป่วยเด็กโรคไข้เลือดออกทั้งในโรงพยาบาลขอนแก่น ครอบครัวและชุมชน ผู้เข้าร่วมวิจัยประกอบด้วยทีมสุขภาพในหอผู้ป่วยเด็ก 25 คน ผู้ป่วยเด็กโรคไข้เลือดออกและครอบครัว 30คู่ และอาสาสมัครแกนนำในชุมชน 20 คน เก็บข้อมูลโดยสัมภาษณ์เชิงลึก การสนทนากลุ่ม การสังเกต และแบบสอบถาม ผลวิจัยพบว่าการพัฒนาศักยภาพประกอบด้วย 1).พัฒนาสมรรถนะทีมสุขภาพโดยใช้การจัดการรายกรณี แนวปฏิบัติการพยาบาลผู้ป่วยเด็กโรคไข้เลือดออกและแผนการจำหน่าย 2). พัฒนาทักษะการดูแลตนเองของผู้ป่วยเด็กและครอบครัว 3). พัฒนาการดูแลต่อเนื่องโดยการมีส่วนร่วมของโรงพยาบาล ครอบครัวและชุมชน ผลการพัฒนาทำให้ผู้เข้าร่วมวิจัยมีความรู้และการปฏิบัติที่ถูกต้องเพิ่มขึ้น ไม่เกิดภาวะแทรกซ้อนภายหลังจำหน่าย ไม่เกิดการแพร่กระจายโรคในชุมชน ความพึงพอใจของผู้เข้าร่วมวิจัยต่อการพัฒนาศักยภาพอยู่ในระดับมาก
Abstract
This action research study focused on developing a hospital- family-community competency for continuing care of pediatric patients with dengue hemorrhagic fever in Khon Kaen hospital. The participants included 25 health care personel of a pediatric ward, 30 pediatric patients and their families, 20 community leader volunteers. In-depth interviews, observation, focus groups and assessment forms of knowledge-attitude- practice were used for gathering data. Result revealed that a hospital-family-community based continuing care depended on1).enhancement of the health care provider competency; case management, clinical nursing practice guideline and discharge planning 2). enhancement of the patient-family’s self-care competency 3). developing continuing care management by hospital-family -community participation. The participants’ knowledge and practice were improved. There was no complication after discharge from hospital and no out-break occurred in community. The participants expressed satisfaction with their continuing care competency at good level.
