Abstract

บทคัดย่อ การดำาเนินโรคของผู้ป่วยไวรัสตับอักเสบซีเรื้อรังแต่ละคนมีความแตกต่างกันมากตั้งแต่ไม่มีอาการจนถึงตับแข็งและมะเร็งตับliverbiopsyเป็นวิธีมาตรฐานในการประเมินผู้ป่วยแต่ก็มีข้อเสียมากจึงได้พยายามหาวิธีการต่างๆเพื่อเป็นวิธีทางอ้อมในการประเมินสภาพการอักเสบและปริมาณพังผืดในตับ วัตถุประสงค์ เพื่อศึกษาความสัมพันธ์ระหว่างระดับซีรั่มHCVviralloadและซีรั่มALTกับความรุนแรงของการอักเสบของตับในผู้ป่วยไวรัสตับอักเสบซีเรื้อรัง วิธีการศึกษา เป็นการศึกษาย้อนหลังจากผลเลือดของผู้ติดเชื้อไวรัสตับอักเสบซีเรื้อรังรายใหม่ของแผนกทางเดินอาหารกองอายุรกรรมโรงพยาบาลพระมงกุฎเกล้าตั้งแต่มกราคมพ.ศ.2552ถึงเมษายนพ.ศ.2555 เปรียบเทียบกับผล liverbiopsy แล้วจึงนำไปหาความสัมพันธ์ ผลการศึกษา ได้จำนวนผู้ป่วยทั้งหมด 20ราย เมื่อนำข้อมูลที่ได้มาทำาการทดสอบความสัมพันธ์ระหว่างผลการตรวจliverbiopsyกับค่าHCVviralloadและผลการตรวจliverbiopsyกับค่าALTแล้วพบว่าไม่มีความสัมพันธ์กันที่ระดับนัยสำคัญ0.05 วิจารณ์และสรุป จากผลการศึกษาพบว่าผลการตรวจทางพยาธิวิทยาของชิ้นเนื้อตับไม่มีความสัมพันธ์กับHCVviralloadและค่าALT

Introduction: Each individual patient with chroniche patitis Cvirus(HCV)infection has various clinical outcomes.The severity of disease is definitely evaluated by the liverbiopsy. However,non-invasive assessment is currently used for evaluation of these patients. Therefore, the aim of this study was to determine the correlation of serologic and virologic tests with thehistopathological changes in liverbiopsy in patients withchronic HCV infection. Materials and Methods: A retrospective study wasconducted at the Gastrointestinal Unit, Division of Internal
Medicine, Phramongkutklao Hospital. The results of serumhepatitisC viral load and alanine aminotransferase(ALT) were compared with histopathological findings on the corresponding liverbiopsyderived from 20 newcases diagnosed as chronic HCV infection from January 2009 to April 2012. Results: At the 95% confidence interval, patients with chronic HCV infection revealed that the serum levels of hepatitis C viralload and ALT were not significantly related to the microscopic appearance of the corresponding livertissue. Conclusion: Inchronic HCV infection, the serum viral load and ALT level were not correlated with the histopathological changes in the liver.
