ผลของการประคบแผลฝีเย็บหลังคลอดด้วยผ้าอนามัยชุบน้ำเกลือเย็น

และผ้าอนามัยชุบน้ำสะอาดเย็น

ต่อ การลดความเจ็บปวด และการบวมของแผลฝีเย็บ

บทคัดย่อ

          การวิจัยนี้เป็นการวิจัยกึ่งทดลอง เพื่อศึกษาเปรียบเทียบความเจ็บปวด และการบวมแผลฝีเย็บของมารดาหลังคลอด ที่ได้รับการประคบแผลฝีเย็บ ด้วยผ้าอนามัยชุบน้ำเกลือเย็น  ผ้าอนามัยชุบน้ำสะอาดเย็น กลุ่มตัวอย่างประกอบด้วย มารดาหลังคลอดในห้องคลอด โรงพยาบาลนครพิงค์ แบ่งออกเป็น 3 กลุ่ม คือ กลุ่มที่ใช้ผ้าอนามัยชุบน้ำเกลือเย็น  ผ้าอนามัยชุบน้ำสะอาดเย็น และกลุ่มที่ไม่ใช้  โดยประคบเย็นหลังเย็บแผลทันที ,ประคบเย็นหลังเย็บแผล 30 นาที(หลังประคบเย็นครั้งที่ 1) และประคบเย็นหลังเย็บแผล 60 นาที (หลังประคบเย็นครั้งที่ 2) ประเมินความเจ็บปวดแผลฝีเย็บโดยใช้แบบประเมินระดับความเจ็บปวด และประเมินอาการบวมแผลฝีเย็บโดยใช้แบบประเมินลักษณะแผลฝีเย็บหลังคลอด (REEDA scale) เก็บข้อมูลระหว่าง 1 ตุลาคม 2556 - 30 ธันวาคม 2556 วิเคราะห์ข้อมูลสถิติพรรณนา คือ ร้อยละ ค่าเฉลี่ย และสถิติวิเคราะห์ได้แก่ Kruskal-Wallis Test กำหนดค่าความเชื่อมั่นที่ระดับนัยสำคัญทางสถิติ .05

          ผลการศึกษา พบว่า กลุ่มที่ใช้ผ้าอนามัยชุบน้ำเกลือเย็น และผ้าอนามัยชุบน้ำสะอาดเย็น ประคบแผลฝีเย็บหลังเย็บแผล หลังเย็บแผล 60 นาที  มีคะแนนความเจ็บปวด น้อยกว่ากลุ่มที่ไม่ใช้การประคบเย็น  แตกต่างอย่างมีนัยสำคัญทางสถิติ (p<.01) ส่วนการประเมินอาการบวมไม่พบการบวมของแผลฝีเย็บ ทั้ง 3 กลุ่ม ดังนั้นควรนำประคบเย็นมาใช้หลังเย็บแผล 60 นาที เพื่อลดปวด และประคบทันทีเพื่อป้องกันการบวมแผลฝีเย็บ ส่งผลให้สุขสบายหลังคลอด

Abstract

This quasi-experimental  research was conducted to compare the Effect of cold compress with cold normal saline pad and cold sterile water pad on perineum pain and edema in post partum  between the cold saline pad, cold sterile water pad  and without cold pad. The simple random of 90 post partum divided into 3 groups used cold saline pad, cold sterile water pad  and without cold pad. Intervention; In the first compress (immediately after stitches), second compress (30 minutes after stitches) and third compresses (60 minutes after stitches). Perineal pain was assessed by the numeric pain rating scale and perineal edema was assessed by REEDA Scale. Collected data was between October 2013,1-December 2013,30.The data was analysed  by descriptive statistics; percentage, average and statistical analysis; Kruskal-Wallis test were given a confidence level of statistical significance of 0.05.

The study found that those who used cold saline pads and cold pad compress perineal reduce pain at 60 minutes after stitches were significantly statistically (p<.01). The assessed of swelling were not swelling all 3 groups. So that it should be cold compress to the wound at 60 minutes after stitches to reduce pain and prevent swelling and erythema of perineal. After birth, the mother is happy to activity diary life.
