Abstract

Many stroke patients are unable to get rehabilitation services after discharge from the hospital.
They might have highly risk to become disability people.The aim of this study was to development of
Intermediate Rehabilitation setting from hospital to community after stroke and to study the results of
the rehabilitation of stroke patient. This research was performed at BuddhachinarajPhitsanulok Hospital
during November 2015-October 2016, in Intermediate Care (IMC) patients(onset less than 6 months
with Barthel Index (BI) < 75) and long term care (LTC) (onset more than 6 months) The study has four
stages: 1 analyzing the old rehabilitation services in community, 2 developing the rehabilitation setting,
3 Implemented the setting designed and evaluation. 4. conclusion. Wilcoxon Signed Ranks Test and
Mann-Whitney U were used and statistically significantat 0.05. The development of the IMC Patient
Rehabilitation Services System in the community of Muang District covers 25 service areas averagely
twice a month by thepatient care team designs. Every day one physiotherapist provides rehabilitation
services in the community with the community team. Set priority of IMC services before LTC. Evaluation of
services daily in the line group has been used. 48 IMC patients and 36LTCs patients which BI < 75
were analyzed. The average numbers of treatment were5.4 and 3.5 times respectively. The median BI of
both group were same at the beginning (BI = 40). At the end of treatment, BI score had increase in both
groups (p < 0.001) but differencein amount which 90 in IMC and 50 in LTC (p < 0.001).In summary,
the development of a community-based intermediate rehabilitation setting in stroke patient enable
patients to access continuous rehabilitation services and improve function.
