The Difference in Length and Cost of Hospital Stay in Patients with Bipolar Disorder Manic with Psychosis Treated with a Mood Stabilizer Combined with Typical and Atypical Antipsychotcs

Abstract

ABSTRACT
Objective : To compare the differences between two groups of patients diagnosed with bipolar affective disorder, currently manic episode with psychotic symptoms who were treated with a combination of mood stabilizer with typical antipsychotics and those who were treated with a combination of mood stabilizers with atypical antipsychotic. The aspects compared included: length of stay in hospital, cost of stay in hospital, and number of admissions.
Methods : A retrospective study of patients who were diagnosed with bipolar affective disorder, currently manic episode with psychotic symptoms who were admitted to Maharaj Nakorn Chiang Mai hospital or Suanprung mental hospital from January 2009 to June 2011 was performed. Information regarding length of hospital stay, cost of hospital stay and number of admissions was found in the medical records. Statistic analysis used independent t-test and chi-square.
Results : Ninety-nine patients were recruited. Patients who received a combination of mood stabilizer with atypical antipsychotic had a 6.34 days shorter length of stay than patients who received a combination of mood stabilizer with atypical antipsychotic with statistically significant at p=0.01. There was a greater cost for the hospital stay for the group with the mood stabilizer combind with atypical antipsychotic medication (7,053.19 Baht, p<0.01).There was no difference in number of admissions between two groups.
Conclusions : Treatment of bipolar affective disorder, currently manic episode with a psychotic symptoms by using a combination of mood stabilizers with atypical antipsychotic may result in a shortened length of hospital stay-in comparison with using a combination of mood stabilizers and typical antipsychotic, but the cost of their hospital stay increased.
