บทคัดย่อ

หลักการและเหตุผล: ภาวะโลหิตจางพบได้บ่อยในโรคมะเร็งปอด ซึ่งภาวะนี้ส่งผลต่อประสิทธิผลการรักษา และคุณภาพชีวิตของผู้ป่วย แต่มะเร็งปอดแบ่งออกเป็นหลายชนิดตามลักษณะทางพยาธิวิทยา ดังนั้นการจำแนกว่าชนิดใดมีผลต่อการเกิดภาวะโลหิตจาง รวมถึงการหาปัจจัยที่เกี่ยวข้องกับกับภาวะโลหิตจางในโรคมะเร็งปอดก่อนเข้ารับการรักษา จะช่วยให้การแก้ไขภาวะนี้มุ่งเน้นไปยังมะเร็งปอดชนิดนั้นๆได้อย่างจำเพาะเจาะจง
วัตถุประสงค์: เพื่อศึกษาความแตกต่างทางพยาธิวิทยาของมะเร็งปอดและปัจจัยที่เกี่ยวข้องกับภาวะโลหิตจางในผู้ป่วยมะเร็งปอดชนิดเซลล์ไม่เล็กก่อนเข้ารับการรักษา
รูปแบบการศึกษา: การศึกษาแบบย้อนหลังโดยเปรียบเทียบประชากรสองกลุ่มเชิงวิเคราะห์
วิธีการศึกษา: รวบรวมข้อมูลผู้ป่วยในโรงพยาบาลสุรินทร์ที่ได้รับการวินิจฉัยว่าเป็นมะเร็งปอดชนิดเซลล์ไม่เล็กจากการตรวจชิ้นเนื้อโดยที่ยังไม่ได้รับการรักษามาก่อน ตั้งแต่วันที่ 1 มกราคม พ.ศ. 2557 ถึง 30 มิถุนายน พ.ศ. 2561 จากฐานข้อมูลทางอิเล็กทรอนิกส์ เพื่อศึกษาลักษณะทางพยาธิวิทยาของมะเร็งปอด ปัจจัยทางคลินิกและการตรวจทางห้องปฏิบัติการกับผลต่อภาวะโลหิตจาง
ผลการศึกษา: ผู้ป่วยมะเร็งปอดชนิดเซลล์ไม่เล็กที่ยังไม่ได้รับการรักษาจำนวน 184 คน แบ่งเป็นกลุ่มที่มีภาวะโลหิตจาง 91 คน และกลุ่มที่ไม่มีภาวะโลหิตจาง 93 คน พบว่าลักษณะทางพยาธิวิทยาของมะเร็งปอดคือ adenocarcinoma squamous cell carcinoma large cell carcinoma และอื่นๆ มีความแตกต่างอย่างมีนัยสำคัญทางสถิติระหว่างกลุ่มที่มีภาวะโลหิตจางกับกลุ่มที่ไม่มีภาวะโลหิตจาง(p=0.011) โดย Squamous cell carcinoma เป็นกลุ่มที่มีภาวะโลหิตจางมากกว่าชนิดอื่น(p=0.004) และ adenocarcinoma เป็นกลุ่มที่ไม่มีภาวะโลหิตจางเป็นส่วนใหญ่ (p=0.003) ส่วนปัจจัยที่เกี่ยวข้องกับภาวะโลหิตจางในมะเร็งปอดชนิดเซลล์ไม่เล็กอย่างมีนัยสำคัญทางสถิติ ได้แก่ albumin < 3.5 g/dl(OR=3.64, 95% CI: 0.14-0.45, p<0.001) และเม็ดเลือดขาว< 5000 cells/uL (OR=6.93, 95% CI:0.01-0.67, p=0.047)
สรุป: ผลตรวจทางพยาธิวิทยาของมะเร็งปอดชนิดเซลล์ไม่เล็ก มีผลต่อการเกิดภาวะโลหิตจางที่แตกต่างกัน โดย squamous cell carcinoma สัมพันธ์กับการเกิดภาวะโลหิตจางมากกว่าชนิดอื่นส่วนปัจจัยที่เกี่ยวข้องกับการเกิดภาวะโลหิตจางในมะเร็งปอดชนิดเซลล์ไม่เล็กก่อนเข้ารับการรักษา ได้แก่ albumin < 3.5 g/dl และ WBC < 5000 cells/uL ซึ่งปัจจัยเหล่านี้สามารถแก้ไขได้ เพื่อเพิ่มประสิทธิภาพขณะรับการรักษา ลดภาวะแทรกซ้อนจากการรับเลือดหรือยากระตุ้นเม็ดเลือด และส่งเสริมคุณภาพชีวิตที่ดีขึ้น
