บทคัดย่อ

บทนำ: แผนกฉุกเฉินมีบริบทการทำงานที่จำเพาะแตกต่างจากส่วนอื่นของโรงพยาบาล กลับยังมีข้อมูลส่วนน้อยที่ศึกษาเกี่ยวกับปัจจัยที่ส่งผลต่อการช่วยฟื้นคืนชีพสำเร็จในผู้ป่วยหัวใจหยุดเต้นภายในแผนกฉุกเฉินที่แยกข้อมูลออกมาอย่างชัดเจน
วัตถุประสงค์: เพื่อศึกษาอัตราและปัจจัยที่มีผลต่อการกลับคืนของระบบไหลเวียนโลหิตที่มากกว่า 20 นาที ภายหลังการช่วยฟื้นคืนชีพ
วิธีการศึกษา: เป็นการศึกษาข้อมูลย้อนหลังในผู้ป่วยที่มีภาวะหัวใจหยุดเต้นภายในแผนกฉุกเฉินโรงพยาบาลบุรีรัมย์ตั้งแต่วันที่ 1 ตุลาคม พ.ศ. 2560 ถึง 30 กันยายน พ.ศ. 2561 วิเคราะห์ข้อมูลโดยใช้โปรแกรมสำเร็จรูป
ผลการศึกษา: จากกลุ่มตัวอย่างทั้งสิ้น 69 รายผลการกลับคืนของระบบไหลเวียนโลหิตที่มากกว่า 20 นาทีคิดเป็นร้อยละ 56.5 ผู้ป่วยเกือบทั้งหมดมีผู้พบเห็นขณะหัวใจหยุดเต้น (ร้อยละ 94.2)ภาวะหลักที่ทำให้หัวใจหยุดเต้นคือ ความดันโลหิตต่ำ(ร้อยละ 49.3) และระบบหายใจล้มเหลว (ร้อยละ 36.2)คลื่นไฟฟ้าหัวใจเริ่มแรกส่วนใหญ่เป็นชนิดที่กระตุ้นด้วยไฟฟ้าไม่ได้(ร้อยละ 95.7) ปัจจัยสำคัญที่ส่งผลต่อการช่วยฟื้นคืนชีพสำเร็จ คือ ระยะเวลาการช่วยฟื้นคืนชีพที่น้อยกว่า 10 นาที(OR: 7.0, 95%CI 2.0-24.2,p = 0.002)
สรุป: ภาวะหลักที่ทำให้หัวใจหยุดเต้นคือ ภาวะความดันโลหิตต่ำและภาวะระบบหายใจล้มเหลว โดยระยะเวลาการช่วยฟื้นคืนชีพน้อยกว่า 10 นาทีเป็นปัจจัยสำคัญที่ส่งผลต่อการช่วยฟื้นคืนชีพสำเร็จ
