Abstract

ที่มาและความสำคัญของปัญหาการวิจัย: การให้คุณค่าและความเชื่อที่ถูกปลูกฝังกันมาตามวัฒนธรรมและความเชื่อทางศาสนามีอิทธิพลต่อการตัดสินใจบริจาคอวัยวะ ประเทศไทยมีชาวไทยมุสลิมจำนวนมากเป็นอันดับสองรองลงมาจากชาวไทยพุทธ ซึ่งพบว่ามีผู้บริจาคอวัยวะน้อยมาก

วัตถุประสงค์: เพื่อศึกษาการให้คุณค่าและความเชื่อของครอบครัวมุสลิมเกี่ยวกับการบริจาคอวัยวะ การตัดสินใจในการบริจาคหรือไม่บริจาคอวัยวะ ความรู้สึกและเหตุผลประกอบการตัดสินใจ

ระเบียบวิธีวิจัย: กลุ่มตัวอย่าง คือครอบครัวของผู้ป่วยมุสลิมที่มีโอกาสเป็นผู้บริจาคอวัยวะ (family members of potential Muslim donor: F-PMD) จำนวน 15 คน รวบรวมข้อมูลเชิงปริมาณโดยการสัมภาษณ์ตามแบบสอบถามการให้คุณค่าและความเชื่อเกี่ยวกับการบริจาคอวัยวะ และข้อมูลเชิงคุณภาพเรื่องการตัดสินใจเกี่ยวกับการบริจาคอวัยวะ ความรู้สึกที่เกิดขึ้นและเหตุผล วิเคราะห์ข้อมูลเชิงปริมาณด้วยสถิติบรรยาย และข้อมูลเชิงคุณภาพด้วยการวิเคราะห์เนื้อหา

ผลการวิจัย: กลุ่มตัวอย่างมีการให้คุณค่าและความเชื่อสูง 3 อันดับแรกในด้านการคล้อยตาม ความปลอดภัย และความเอื้อเฟื้อเผื่อแผ่ ตามลำดับ ส่วนใหญ่ไม่สนใจที่จะบริจาคอวัยวะของผู้ป่วย (n=12) โดยระบุเหตุผลว่าอยากให้ผู้ป่วยตายไปโดยมีอวัยวะครบทุกส่วนตามหลักของพระเจ้า และระบุว่าไม่สามารถตัดสินใจเพียงคนเดียวได้ต้องหารือกันในครอบครัว

สรุปและข้อเสนอแนะ: บุคลากรผู้ทำหน้าที่เป็นผู้เจรจาขอบริจาคอวัยวะ ควรประเมินและทำความเข้าใจกับระบบความคิดด้านการให้คุณค่าและความเชื่อของครอบครัวผู้ป่วยมุสลิม เพื่อให้กระบวนขอบริจาคอวัยวะราบรื่น

Background and Significance of the Problem: Values and beliefs embedded through cultures and religious beliefs influence how people make choices for organ donation. In Thailand, Muslims are the second largest religious group after Buddhists. It has also been reported that the number of Muslim organ donors was low.

Objective: To explore values and beliefs regarding organ donation among families of potential Muslim organ donors (F-PMD), their decisions to donate or not donate organs and its rationale.

Methods: Fifteen family members of F-PMD comprised the sample. Quantitative data were collected through interview as structured by the Values and Beliefs Organ Donation-Family version (VBOD-F) Questionnaire. Qualitative data were collected using open-ended questions asking the decision they would make if they were asked to donate organs of the patients, their own feelings if they make such decision and the rationale. Quantitative and qualitative data were analyzed using descriptive statistics and content analysis, respectively.

Results: The top three domains of values and beliefs regarding organ donation that were rated the highest, in order, were conformity, security, and benevolence. The majority (n=12) reported not to donate organs of their patients who would be determined as having brain death. The major reason was that the patient shall die with full body integrity as Allah’s wish. In addition, they were unable to independently make the decision but they need to discuss with other family members.

Conclusion and Recommendations: Healthcare workers should assess and understand values and beliefs system of Muslim families in order to facilitate the smooth process of organ donation procurement.
