THE EFFECT OF THE COGNITIVE REHABILITATION PROGRAM ON DEPRESSION AMONG OLDER ADULTS WITH COGNITIVE IMPAIRMENT

Abstract

          This study was based on a 2-group quasi-experimental research design with pre-and post-test measurements. The objective was to study the effects of a cognitive rehabilitation program on depression among older adults with cognitive impairment. The sample was composed of 60 older adults with cognitive impairment evaluated by Montreal Cognitive Assessment (MoCA) scores of less than 25 points, no severe communicable illnesses, no uncontrollable chronic diseases and the ability to communicate in the Thai language. The sample was arranged in matched pairs to ensure that the two groups would have similar characteristics in terms of gender, age and cognitive impairment score levels in experimental and control groups of 30 subjects each. The experimental group received the cognitive rehabilitation program created by the researcher based on the cognitive rehabilitation concept of Huckans and colleagues and 4 stages of the problem solving style concept of D’Zurilla and Nezu. The program was offered in 8 weekly sessions lasting for 90-120 minutes each over a period of 8 consecutive weeks. The control group received routine care only. The instrument employed in the research included the cognitive rehabilitation program, which was tested for content validity by three qualified professionals and the Cornell Scale for Depression in Dementia (CSDD), which earned a Cronbach’s alpha coefficient score of 0.74. Data was analyzed by using descriptive statistics, the Wilcoxon Signed Ranks Test and the Mann-Whitney U Test.


          The research findings were as follows:  1. The post-test depression score for the older adults with cognitive impairment in the experimental group (Mean Rank = 0.00) was lower than the pre-test score (Mean Rank = 15.5) with statistical significance (p <.05). 2. The mean depression scores for the older adults with cognitive impairment after receiving the cognitive rehabilitation program indicated that the experimental group (Mean Rank = 16.90) had lower scores than the control group receiving routine care only (Mean Rank = 44.10) with statistical significance (p<.05).
