บทคัดย่อ

          การวิจัยนี้เป็นการวิจัยเชิงคุณภาพ มีวัตถุประสงค์เพื่อวิเคราะห์ทิศทางและกระบวนการพัฒนาระบบสุขภาพอำเภอชายแดนไทย-ลาว โดยใช้การสนทนากับกลุ่มเป้าหมายประกอบด้วยผู้นำภาคีเครือข่ายที่มีส่วนสำคัญในการพัฒนาระบบสุขภาพ อำเภอชายแดนไทย-ลาว ใน 6 อำเภอ ๆ ละ 10 คน รวมทั้งสิ้น 60 คน โดยแบ่งออกเป็นระดับอำเภอ 42 คน และระดับจังหวัด 18 คน ด้วยแนวคำถามที่สร้างขึ้นตามแนวคิดการพัฒนาระบบสุขภาพอำเภอ1 เก็บรวบรวมข้อมูลระหว่างวันที่ 1 มกราคม 2558-28 กุมภาพันธ์ 2561 นำข้อมูลที่ได้มาวิเคราะห์เชิงเนื้อหา

          ผลการวิจัย พบว่า ทิศทางและกระบวนการพัฒนาระบบสุขภาพอำเภอชายแดนไทย-ลาว ประกอบด้วย 6 ด้าน คือ 1) ด้านการทำงานร่วมกันในระดับอำเภอมีการบริหารจัดการที่เข้มแข็งและข้อตกลงร่วมกันของคณะกรรมการทุกภาคส่วนในพื้นที่ 2) ด้านการมุ่งผลลัพธ์ที่เกิดกับผู้บริโภคมีเพิ่มช่องทางสื่อสารและมีการแจ้งข่าวสารในขณะให้บริการทั้งการพบปะพูดคุยและการสื่อสารโดยใช้กลุ่มไลน์ 3) ด้านการมีส่วนร่วมของชุมชนและเครือข่าย หน่วยงานสาธารณสุข องค์กรปกครอง ส่วนท้องถิ่น ผู้นำชุมชน และอาสาสมัครสาธารณสุขประจำหมู่บ้านมีส่วนร่วมทุกขั้นตอน และภาคีอื่นทำงานที่มีลักษะการเชื่อมโยงแบบหลวมๆ เข้าร่วมบางกิจกรรม และภาคเอกชนเริ่มมีส่วนร่วมมากขึ้น 4) ด้านการเห็นคุณค่าในการทำงานของผู้ให้บริการและผู้รับบริการ โดยเริ่มมีการปรับแนวคิดและทัศนคติในการทำงานด้วยใจของผู้นำและทีมลงพื้นที่เยี่ยมประชาชนถึงบ้าน โดยการเอาใจใส่เสียสละทุ่มเทสมํ่าเสมอ 5) ด้านการแบ่งปันทรัพยากรและพัฒนาบุคลากร โดยมีองค์กรปกครองส่วนท้องถิ่นเข้ามามีส่วนร่วมสมทบเพิ่มมากขึ้น 6) ด้านการให้บริการสุขภาพตามความจำเป็นในบริบทของชุมชน โดยมีการดูแลคุณภาพชีวิตผู้สูงอายุ ผู้ป่วยติดบ้านติดเตียง ผู้ป่วยโรคเรื้อรังผู้ด้อยโอกาสทางสังคมและภาวะฉุกเฉินเมื่อเกิดโรคระบาด โดยมีข้อเสนอเชิงนโยบายว่าภาครัฐควรสนับสนุนทรัพยากรสำหรับระบบสุขภาพอำเภอเพื่อให้เกิดการแก้ปัญหาได้อย่างเบ็ดเสร็จ ส่งเสริมการพัฒนาภาวะผู้นำให้แก่ภาคีเครือข่ายเพื่อสร้างแรงบันดาลใจให้ทำงานด้วยหัวใจความเป็นมนุษย์ร่วมกันพัฒนาคุณภาพชีวิตของคนในพื้นที่ที่สอดคล้องกับวิถีวัฒนธรรมชุมชนท้องถิ่นแบบไร้รอยต่ออย่างยั่งยืน
