บทคัดย่อ

      การวิจัยเชิงคุณภาพ (qualitative research) ครั้งนี้มีวัตถุประสงค์เพื่อศึก ษานโยบายและการดูแลสุขภาพ เด็กติดผู้ต้องขังตามการรับรู้ของมารดาในเรือนจำ เก็บข้อมูลโดยการสัมภาษณ์เชิงลึกผู้ต้องขังหญิงที่เป็นมารดา จำนวน 30 ราย การสังเกตสภาพแวดล้อมในเรือนจำ และการทบทวนเอกสารที่เกี่ยวข้องกับการดูแลสุขภาพเด็กติดผู้ต้องขัง เก็บรวบรวมข้อมูลระหว่าง ตุลาคม 2555 ถึง เมษายน 2556 ข้อมูลที่ได้จากการ ถอดเทปนำมาวิเคราะห์เนื้อหา

     ผลการวิจัย พบการรับรู้ข้องมารดาเด็กติดผู้ต้องขังใน 2 ประเด็นหลัก ดังนี้ 1. กำหนดนโยบายให้เอื้อเพื่อ ประโยชน์ต่อเด็กติดผู้ต้องขัง ประกอบด้วย 6 ประเด็นย่อยคือ 1.1 ให้เลี้ยงลูกได้หนึ่งปี มียึดหยุ่นตามสภาพ 1.2 สวัสดิการของใช้เด็กจัดให้เพียงพอระยะแรก หลังจากนั้นให้ตามงบประมาณ 1.3 ให้เลี้ยงลูกด้วยนมแม่ และดูแลลูกด้วยตนเอง 1.4 จัดให้มีพี่เลี้ยงเด็กอาสา ช่วยมารดาเลี้ยงดูลูก 1.5 จัดพื้นที่ให้เหมาะกับการ เลี้ยงเด็กเท่าที่สถานที่จะเอื้ออานวย และ 1.6) มีมาตรการลดคนเข้าใกล้ เพื่อไม่ให้เด็กติดเชื้อ; และ 2. ดูแล สุขภาพเด็กอยู่ข้างใน(เรือนจา) ให้ใกล้เคียงเด็กอยู่ข้างนอก ประกอบด้วยประเด็นการดูแลสุขภาพเด็ก ติดผู้ต้องขัง 4 ประเด็น ได้แก่ 2.1 มีการประเมินและส่งเสริมการเจริญเติบโตด้วยอาหารตามวัย 2.2 มีการ ประเมนิ และสง่ เสรมิ พฒั นาการตามสภาพ 2.3สร้างเสริมภูมิ คุ้มกันด้วยวัคซีน เจ้าหน้าที่นำออกไปหรือจัด ให้ในเรือนจำ และ 2.4 แม่ดูแลเบื้องต้นเมื่อเจ็บป่วย เจ้าหน้าที่ช่วยและส่งโรงพยาบาลเมื่ออาการรุนแรง

     ข้อเสนอแนะ จากการวิจัย เนื่องจากทั้งนโยบายและการดูแลเด็กของเรือนจำนั้นเน้นให้มารดาเป็นผู้เลี้ยง บุตรด้วยตนเอง ดังนั้นจึงควรจัดอบรมเพื่อพัฒนาความรู้และทักษะการเลี้ยงดูเด็กแก่มารดาและพี่เลี้ยงเด็ก อาสาสมัครอย่างต่อเนื่องเพื่อให้เด็กได้รับการเลี้ยงดูอย่างถูกต้อง รวมทั้งแสวงหาเครือข่ายและสร้างความร่วมมือกับองค์กรภายนอกเพื่อขอการสนับสนุนเชิงวิชาการ หรือ เงินทุน สิ่งของเครื่องใช้ที่จาเป็นสำหรับเด็ก ติดผู้ต้องขัง

  Abstract

     This qualitative research aimed to study policies and health care practice for children perceived by inmate mothers. Thirty inmate mothers, who were raising their child in the prison environment, were in-death interviewed. Field observation in the prison was included and documents related to health care practice for children in the prison were reviewed. Data collection period was from October 2012 to April 2013. Data from the interview were transcribed and analyzed by using content analysis.

     The results showed two major themes emerged from the data: 1. “Setting policies for the children’s benefits” including 6 sub-themes: 1.1 allowing the child to stay in the prison for only one year, but being flexible in some cases; 1.2 providing enough baby accessories after birth, after that depend on the budget; 1.3) encouraging the mother to care and breastfeed their own child; 1.4) arranging baby sister volunteers to help babysit the child; 1.5) arranging area suitable for raising children as much as possible, and 1.6) prohibiting other prisoners to approach the child to prevent infection; and 2. “Provide health services to the children inside as good as those outside” embracing 4 health services for the children: 2.1 evaluating and promoting child growth with appropriate foods; 2.2 evaluating and promoting child development as circumstance; 2.3 vaccinating at inside or outside the prison; and 2.4 providing primary care by mothers, then helping or transferring to hospital outside by the officer in case of severe illness.

     The result from this study suggested that training program to improve knowledge and skills of inmate mothers and baby sister volunteers should be provided regularly as well as seeking supports from organization outside for academic or financial support would resolve budget shortage problem in prison and improve quality of care for children with inmate parents.
