การวิจัยเชิงสำรวจระยะยาวครั้งนี้ มีวัตถุประสงค์เพื่อประเมินผลกระทบของการขึ้นภาษีสรรพสามิตยาสูบเมื่อ 21 สิงหาคม 2555 จากร้อยละ 85 เป็น 87 ที่มีต่อพฤติกรรมการบริโภคยาสูบของผู้บริโภคยาสูบปัจจุบัน เก็บรวบรวมข้อมูลด้วยการสัมภาษณ์ทางโทรศัพท์ในกลุ่มตัวอย่างผู้สูบบุหรี่ปัจจุบันที่มีอายุตั้งแต่ 15 ปีขึ้นไปจากฐานข้อมูลในโครงการสำรวจการบริโภคยาสูบในผู้ใหญ่ระดับโลก (Global Adult Tobacco Survey: GATS) ปี 2554 ที่มีหมายเลขโทรศัพท์ที่สามารถติดต่อได้ จำนวน 1,299 คน โดยดำเนินการสัมภาษณ์ 3 รอบ คือ หลังการขึ้นภาษี 1, 3 และ 6 เดือน ข้อมูลนำมาวิเคราะห์และเปรียบเทียบด้วยสถิติเชิงพรรณนา ได้แก่ จำนวน ร้อยละ ค่าเฉลี่ย และส่วนเบี่ยงเบนมาตรฐาน

ผลการศึกษาพบว่า หลังการขึ้นภาษีสรรพสามิตยาสูบครั้งนี้ นาน 1 เดือน มีผลให้ผู้สูบบุหรี่ปัจจุบันที่เลิกสูบบุหรี่ได้ ร้อยละ 2.0 ในจำนวนนี้ เลิกสูบบุหรี่เพราะเหตุผลของการขึ้นภาษี ร้อยละ 12.5 และมีผู้สูบบุหรี่

เป็นประจำทุกวันลดลง สำหรับปริมาณบุหรี่โรงงานที่สูบ พบว่ามีแนวโน้มลดลงเล็กน้อยจาก 10.4 (± 6.7)

มวนต่อวันต่อคน เป็น 9.3 (± 7.2) – 9.4 (±6.2) มวนต่อวันต่อคน ส่วนปริมาณบุหรี่มวนเองที่สูบเท่ากับ

11.1 (± 7.5) มวนต่อวันต่อคน ลดลงเป็น 9.0 (± 7.5) – 9.3 (±6.9) มวนต่อวันต่อคน ขณะเดียวกัน

พบว่ามีการเปลี่ยนแปลงการสูบบุหรี่ทั้งบุหรี่โรงงานและบุหรี่มวนเอง มาเป็นการสูบอย่างเดียวมากขึ้น พบการ

เปลี่ยนยี่ห้อบุหรี่ ร้อยละ 15.3 เปลี่ยนประเภทผลิตภัณฑ์ ร้อยละ 7.7 และสูบบุหรี่โรงงานราคาถูกเพิ่มขึ้น

ดังนั้น กระทรวงสาธารณสุข จึงควรร่วมมือกับกรมสรรพสามิต กระทรวงการคลังศึกษาระบบและ

โครงสร้างภาษีที่เหมาะสมที่มีผลทั้งต่อราคาบุหรี่ และการเลิกสูบบุหรี่ พร้อมทั้งสถานบริการสาธารณสุขและ

ภาคีเครือข่ายในทุกระดับ ควรปรับรูปแบบการทำงาน โดยเฉพาะบริการเลิกบุหรี่ให้เป็นเชิงรุกและพัฒนา

โปรแกรมสื่อสารสุขภาพเพื่อการช่วยเลิกบุหรี่ให้มีประสิทธิภาพยิ่งขึ้น

 

Abstract

This longitudinal research aimed to assess an impact of tobacco excise tax increase in 2012, August, from 85% to 87% on tobacco consumption behavior among current tobacco users. The data were collected by telephone interview among samples who were current tobacco users aged 15 years and over. Samples were 1,299 tobacco users from Global Adult Tobacco Survey (GATS) in 2011 and have had telephone number. Interview has been done for 3 times, after tax increased for 1, 3 and 6 months. Comparative data analysis was descriptive statistics such as frequency, percentage, mean, and standard deviation.

The results indicated that 2% of current smokers who quit after one month of tobacco tax increase. Some of these were quit smoking with the reason of raising tax and cigarette prices for 12.5% and reduced number of daily smokers. For amount of cigarette, the finding showed the a slightly downward trend of manufactured cigarette from 10.4 (±6.7) sticks per day to 9.3 (±7.2)–9.4 (±6.2) sticks per day and the same as hand rolled cigarettes that decreased from 11.1 (± 7.5) sticks per day to 9.0 (± 7.5) – 9.3 (±6.9). Moreover, there were smoking behavior change from smoking two types, both manufactured and hand rolled cigarette to single type. The changes were 15.3% of brand change, 7.7% of product change, and higher smoking cheap brand cigarettes produced by domestic manufacture for 7% while

imported cigarette smoking was quite stable. Recommendations to Ministry of Public Health and Department of Excise Tax, Ministry of Finance should be cooperated to study and propose suitable excise tax structure and system that have affected on both raising price and quit smoking. Health services and all tobacco control networks should adapt the cessation service from in-office service to proactive service for smokers to easily access and including developing health communication program for help to quit smoking effectively.
