งบจ่ายตามเกณฑ์ศักยภาพ (On top payment) เป็นงบเพื่อกระตุ้นการพัฒนาศักยภาพหน่วยบริการ

ปฐมภูมิ (PCU) อย่างต่อเนื่อง และส่งเสริมการใช้บริการใกล้บ้านใกล้ใจของประชาชน ปัญหาของการตรวจประเมินเพื่อขอรับงบประมาณนี้ คือ มีการตีความหมายของเกณฑ์แตกต่างกัน กรรมการขาดรูปแบบการตรวจประเมินที่ชัดเจนเป็นไปในทางเดียวกัน และยังพบข้อผิดพลาดจากการตรวจประเมิน ส่งผลให้เกิดความขัดแย้งระหว่างหน่วยงานและลดความน่าเชื่อถือของกระบวนการประเมินและผู้ประเมิน การวิจัยครั้งนี้เป็นวิจัยเชิงปฏิบัติการ เพื่อพัฒนาและศึกษาผลลัพธ์ของการใช้รูปแบบการตรวจประเมินหน่วยบริการปฐมภูมิแบบเสี่ยวโมเดล ระหว่าง ปีงบประมาณ 2554 ถึง 2556 กลุ่มตัวอย่างเป็น ผู้ตรวจประเมินภายใน ผู้ตรวจประเมินภายนอก และคณะกรรมการพัฒนาหน่วยบริการปฐมภูมิทั้ง 7 จังหวัด ได้แก่ อุดรธานี หนองคาย หนองบัวลำภู เลย สกลนคร นครพนมและบึงกาฬ เครื่องมือที่ใช้เป็นแบบประเมินของโรงพยาบาลแม่ข่าย (CUP profile) แบบประเมินของหน่วยบริการปฐมภูมิ (PCU profile) แนวคำถามเชิงโครงสร้างในการสนทนากลุ่มสำหรับคณะกรรมการเครือข่ายหน่วยบริการปฐมภูมิ และแบบสอบถามความคิดเห็นต่อเกณฑ์ประเมินค่าความเชื่อมั่นของครอนบาค เท่ากับ 0.70 วิเคราะห์และสังเคราะห์รูปแบบการตรวจประเมินโดยใช้สถิติร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน และการวิเคราะห์เนื้อหา

     ผลการวิจัย เสี่ยวโมเดล เป็นรูปแบบการตรวจประเมินหน่วยบริการปฐมภูมิ โดยใช้กระบวนการมีส่วนร่วม 3 ขั้นตอน คือ 1) ส่งเสริม พัฒนา 2) สร้างศรัทธา รักษามาตรฐาน 3) รายงานถอดบทเรียน ซึ่งผลลัพธ์ของการใช้รูปแบบนี้ ทำให้มีกระบวนการตรวจมีขั้นตอนชัดเจน มีคู่มือการตรวจประเมินหน่วยบริการปฐมภูมิ ที่สามารถนำไปพัฒนาทีมตรวจประเมินได้ และ มีข้อมูลสารสนเทศเพื่อการจัดสรรงบที่ถูกต้อง ครบถ้วน เป็นธรรม โปร่งใสและตรวจสอบได้ ซึ่งผลลัพธ์การตรวจประเมิน พบว่า ร้อยละของหน่วยบริการปฐมภูมิที่ผ่านเกณฑ์ศักยภาพ ในพื้นที่รับผิดชอบของ สปสช. เขต 8 เพิ่มขึ้นจากร้อยละ 58.54 ในปี 2553 เป็นร้อยละ 98.94 ในปี 2556 สะท้อนให้เห็นถึง ขั้นตอนการพัฒนารูปแบบการตรวจประเมิน แบบมีส่วนร่วมช่วยให้เกิดการบริหารจัดการที่มีประสิทธิภาพ และเป็นแนวทางการประเมินที่นำไปใช้ประโยชน์ให้กับเขตอื่นได้

 

Abstract

On-top payment is an additional budget that the National Health Security Office (NHSO) has set up specific criteria for developing capacities and services of primary care units (PCUs). The main problems of the on-top criteria were the different interpretations of indicators, pattern of assessment, and error in checking PCU profiles. This action research aimed to develop the model for assessing primary care units according to the quality criteria of primary care units and to evaluate the outcome of the model use between a fiscal year 2011 to 2013. Data were collected from internal auditors, external auditors, and key persons in primary care unit development committee from 7 provinces of National Health Security Office (NHSO) region 8 including: Udonthani, Nongbua Lampoo, Nongkhai, Sakonnakhon, Nakhonphanom, Bungkan and Loei. The instruments were the interviewed guide questions for a focus group and questionnaires about opinion on the indicators. The Cronbach’s alpha was 0.70. Data were analyzed and synthesized from CUP profile and PCU profile since 2011 to 2013. Descriptive statistic and content analysis were used to analyze the data.

     The results revealed that: 1. the participation model was an evaluation process that composed of 3 steps: 1) enhancing auditors’ capacities to ensure their understanding on the quality criteria and the indicators, 2) achieving and maintaining the quality performance, and 3) reporting the lesson learnt from assessing primary health care units. 2. The outcome of using the participation model was having the clear steps to follow. The model handbook was provided to be a guideline for developing the auditors’ capacities. The most important outcome was having the accurate data to ensure that the on top payment budget was given with fairness and transparency. 3. This participation model was effective to improve the number of primary care units, under the National Health Security Office (NHSO) region 8, which passed the on top payment criteria from 58.54% in 2010 to 98.94% 2013. Therefore, the participation model reflected effective management of the evaluating process. NHSO region and NHSO of each province should apply this model as a guideline for assessing primary health care units.
