                การวิจัยนี้เป็นการวิจัยเชิงสหสัมพันธ์ (Correlational research) เพื่ออธิบายปัจจัยสำคัญที่มีความสัมพันธ์ กับพฤติกรรมการกินสมุนไพรเพื่อควบคุมระดับน้าตาลในเลือดของผู้ป่วยเบาหวานชนิดที่ 2 ในพื้นที่ตำบล บางหลวง อาเภอบางเลน จังหวัดนครปฐม โดยใช้ PRECEDE model เป็นกรอบกำหนดตัวแปรอิสระ ได้แก่ ปัจจัยนำ ปัจจัยเอื้อและปัจจัยเสริม เก็บรวบรวมข้อมูลในประชากรผู้ป่วยเบาหวานจำนวน 111 คน ด้วยแบบ สัมภาษณ์ วิเคราะห์ความสัมพันธ์ด้วยสถิติไคสแควร์ (Chi-square test) และทำนายพฤติกรรมด้วยสถิติ Binary Logistic regression

                ผลการวิจัย พบว่า ผู้ป่วยเบาหวานชนิดที่ 2 ร้อยละ 55.9 มีการกินสมุนไพรเพื่อลดระดับน้ำตาลในเลือด โดยเกือบทั้งหมด ร้อยละ 98.4 กินสมุนไพรควบคู่กับการกินยาแผนปัจจุบัน และร้อยละ 32.3 ลดการกินยาแผนปัจจุบัน สำหรับปัจจัยที่มีความสัมพันธ์กับพฤติกรรมการกินสมุทรไพร มีดังนี้ ปัจจัยนำ ได้แก่ อายุ (p<.05) ระยะเวลาที่ป่วยเป็นโรคเบาหวาน (p<.01) ฐานะทางเศรษฐกิจ (p=.03)ปัจจัยเอื้อ ได้แก่การมียาสมุน ไพร ในท้องถิ่น (p<.01) และปัจจัยเสริมได้แก่การสนับสนุนของเพื่อนผู้ป่วย (p=.02)ในขณะที่ไม่พบความสัมพัน ธ์ กับระดับการศึกษา การรับรู้ความรุนแรงของโรคเบาหวาน ความพึงพอใจต่อการกินยาแผนปัจจุบัน ความรู้ เรื่องสมุนไพรลดระดับน้าตาลในเลือด การเข้าถึงแหล่งจาหน่ายยาสมุนไพร การสนับสนุนของบุคคลใน ครอบครัวการสนับ สนุน ของเพื่อนบ้านและการสนับสนุน ของเจ้า หน้าที่มากกว่านั้น พบว่า 2ปัจจัย ได้แก่ ฐานะ ทางเศรษฐกิจและระยะเวลาที่ป่วยนานกว่า 10 ปีสามารถทานายพฤติกรรมการกินสมุนไพรได้ร้อยละ 22.4 (R2=0.224)

     ดังนั้น โรงพยาบาลชุมชนและโรงพยาบาลส่งเสริมสุขภาพตำบลควรจัด ให้ความรู้ ความเข้า ใจเรื่องการใช้สมุนไพรลดระดับน้ำตาลในเลือดที่ถูกต้องแก่ผู้ป่วยเบาหวาน กระทรวงสาธารณสุขและหน่วยงานที่เกี่ยวข้อง ควรควบคุมธุรกิจสมุนไพรให้มีการผลิตอย่างปลอดภัย และควรมีการศึกษาวิจัยสมุนไพรเพื่อนามาใช้รักษา ร่วมกับยาสมัยใหม่

Abstract

     This correlational study aimed to explore relationships of factors related to herbal consumption for controlling blood sugar of patients with type 2 diabetic mellitus at Bang Luang Sub-district, Bang Len District, Nakornpathom Province. The PRECEDE model was applied to construct conceptual framework of independent variables include predisposing, enabling and reinforcing factors. Data were collected in population of 111 diabetes mellitus patients by using the interview questions. Chi-square and binary logistic regression were employed to test association among variables.

     Results showed that 55.9 percent of patients with type 2 diabetic mellitus have eaten herbs to decrease blood sugar levels. Almost all of them (98.4 %) took herbs with modern medicine. Among this, 32.3 percent had reduction in consumption of modern medicine. The factors associated with herbal consumption included predisposing factor of age (p < .05), duration of diabetes mellitus (p < .01), economic status (p = .03), the enabling factor include the available of herbs in local areas (p < .01), and the reinforcing factor include support of fellow patients with diabetic mellitus (p = .02). On the other hand, the educational levels, perception of severity of diabetes, patients’ satisfaction of taking drugs, knowledge about herbs to control blood sugar, an accessibility to sources of herbs, support from families, support from neighbors, and support from public health personnel were not associated to herbal consumption. However, duration of diabetes mellitus and economic status accounted for 22.4% of herbal consumption (R2=0.224).

     Therefore, Community Hospital and Tumbol Health Promoting Hospital should deliver knowledge inrelation to proper use of herbs for patients with type 2 diabetic mellitus for reducing blood sugar levels. Moreover, Ministry of Public Health and related agencies should control herbal business to produce safety herbs and should encourage more research of herbs for complementary use with modern medicine.
