บทคัดย่อ


ความเป็นมา


          ภาวะติดเชื้อในกระแสโลหิต (Sepsis) เป็นภาวะที่มีแนวโน้มเพิ่มมากขึ้น และเป็นสาเหตุการเสียชีวิตอันดับ 1 ของผู้ป่วยในโรงพยาบาล ปัจจุบันมีการใช้แนวปฏิบัติการดูแลผู้ป่วยที่มีภาวะติดเชื้อในกระแสโลหิต โดยทีมสหสาขาวิชาชีพ การประยุกต์ใช้แนวปฏิบัติอย่างมีประสิทธิภาพ สามารถทำให้มีอัตราการเสียชีวิตลดลง


วัตถุประสงค์


          เพื่อเปรียบเทียบผลลัพธ์ของการใช้แนวปฏิบัติและคุณภาพด้านการปฏิบัติการดูแลผู้ป่วยที่มีภาวะติดเชื้อในกระแสโลหิต


วิธีการศึกษา


          เป็นการวิจัยเชิงวิเคราะห์ โดยเก็บข้อมูลย้อนหลังในผู้ป่วยที่มีภาวะติดเชื้อในกระแสโลหิตที่เข้ารับการรักษาในโรงพยาบาลฝาง จังหวัดเชียงใหม่ ตั้งแต่เดือนตุลาคม 2559 ถึงเดือนกันยายน 2560 จำนวน 246 ราย เปรียบเทียบก่อนและหลังใช้แนวปฏิบัติ คือ ช่วงตุลาคม 2559 ถึงมีนาคม 2560 และเมษายน 2560 ถึงกันยายน 2560 รวบรวมข้อมูลจากแบบบันทึกข้อมูลเวชระเบียนผู้ป่วย แบบบันทึกข้อมูลการดูแลผู้ป่วยที่มีภาวะติดเชื้อในกระแสโลหิตวิเคราะห์ข้อมูลโดยใช้สถิติพรรณนาและสถิติ Chi-square test, Fisher Exact test และ Independent t test


ผลการศึกษา


          ผู้ป่วยที่มีภาวะติดเชื้อในกระแสโลหิต 246 ราย แบ่งเป็นกลุ่มก่อนใช้แนวปฏิบัติ 85 ราย และกลุ่มหลังใช้แนวปฏิบัติ 161 ราย พบว่า อัตราการเสียชีวิตผู้ป่วยที่มีภาวะติดเชื้อในกระแสโลหิตลดลงจากร้อยละ 28.2 เป็นร้อยละ 21.1 โดยไม่มีนัยสำคัญทางสถิติ ในขณะที่อัตราการส่งเพาะเชื้อก่อนให้ยาปฏิชีวนะ และอัตราการได้รับยาปฏิชีวนะ ภายใน 1 ชั่วโมงหลังได้รับการวินิจฉัยลดลงอย่างมีนัยสำคัญทางสถิติ  รวมทั้งอัตราการได้รับสารน้ำ 30 ml/kg ใน 1 ชั่วโมงแรก และอัตราการดูแลแบบภาวะวิกฤตภายใน 3 ชั่วโมงลดลงแต่ไม่มีนัยสำคัญทางสถิติ


สรุปผลและข้อเสนอแนะ


          การใช้แนวปฏิบัติการดูแลผู้ป่วยที่มีภาวะติดเชื้อในกระแสโลหิต สามารถลดอัตราการเสียชีวิตได้แต่ผลลัพธ์ด้านกระบวนการปฏิบัติกลับลดลง บ่งชี้ว่าการดูแลตามแนวปฏิบัติยังไม่สมบูรณ์ตามเป้าหมายซึ่งต้องพัฒนาต่อไป
