The Development of Health Care Model for the Elderly with Knee Osteoarthritis by Community Participation of Non Sa-ad Health Promoting Hospital, Nong Ruea District, Khon Kaen Province

Abstract

This participatory action research aimed to investigate the contexts related to health care problems, and develop as well as evaluate health care model for the elderly with knee osteoarthritis by community participation of Non Sa-ad Health Promoting Hospital, Nong Ruea District, Khon Kaen Province. Participants: for investigating the contexts related to health care problems of the elderly, were 490 elderly persons; for developing the model, were 24 persons covering community leaders, municipal members, non-formal and informal education officers, local governors, public health officers and health village volunteers; for implementing the model, were 33 elderly persons with knee osteoarthritis and 33 their care givers. The development of health care model was developed in two circles using Plan-Act-Observe-Reflect process. Round 1 was conducted between January- June 2017 and Round 2 was conducted between July 2017 and March 2018. Quantitative data were analyzed by descriptive statistics, including frequency, percentage, mean and standard deviation and also Paired t-test. Quantitative data were analyzed by content analysis.

The results revealed that the developed model was called “LUCKY”. It included L=Learning (learning together between the community and the research team), U=Unity (working together as a team), C=Care (having follow up, home visit and continue empowerment), K=Keep in touch (reflecting the results of the implementation) and Y=Yardstick (developing the model to be standard). The effects of the developed model led to decrease knee pain of the participants significantly (p=0.05) after the model implementation (the highest pain score was 4 and the lowest score was 0, the average score=1.27 and S.D.=0.97), when comparing to before the model implementation (the highest score=10 and the lowest score = 4, the average score=7.12 and S.D.=2.28), the different score=5.85 (95% CI:1.07–1.71 score:p-value<0.001). In addition, the participants were highly satisfied to the model activities (95.5%) and the community participation to the project was at a high level (86.4%). Therefore, community participation and also using Thai local herbs to decrease pain should be recognized to provide a supportive care of the elderly with osteoarthritis for promoting their quality of life.
