ภาวะติดนิโคตินกับอาการทางจิตในผู้ป่วยจิตเภท

บทคัดย่อ

วัตถุประสงค์: เพื่อศึกษาความชุกของภาวะติดนิโคตินในผู้ป่วยจิตเภท ความสัมพันธ์ระหว่างภาวะติดนิโคตินกับระดับความรุนแรงของอาการทางจิต และปัจจัยที่เกี่ยวข้อง

วิธีการ: เป็นการศึกษาภาคตัดขวาง ณ แผนกผู้ป่วยนอก สถาบันกัลยาณ์ราชนครินทร์ เดือนธันวาคม 2561 ถึงมีนาคม 2562 กลุ่มตัวอย่างคือผู้ป่วยโรคจิตเภท 193 ราย ประเมินระดับความรุนแรงของอาการทางจิต โดยใช้ Positive and negative syndrome scale (PANSS) ฉบับภาษาไทย กลุ่มที่มีภาวะติดนิโคตินได้รับการประเมินระดับความรุนแรงของภาวะติดนิโคติน โดยใช้ Fagerstrom test of nicotine dependence (FTND) ฉบับภาษาไทย วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา Chi-square test, unpaired t-test และ Mann-Whitney U test

ผล: ความชุกของภาวะติดนิโคตินในผู้ป่วยจิตเภทเท่ากับร้อยละ 34.2 (95% CI 27.54-41.36) ค่าเฉลี่ย FTND เท่ากับ 4.53  (SD=2.08) ปัจจัยที่สัมพันธ์กับภาวะติดนิโคติน (p-value ≤ 0.05) ได้แก่ เพศชาย อายุที่น้อยกว่า ระดับการศึกษาต่ำกว่าปริญญาตรี การมีอาชีพ การใช้สารเสพติดอื่น ระดับความรุนแรงของอาการทางจิต การไม่ได้รับยาต้านโรคจิตกลุ่มใหม่ (ไม่รวม clozapine) เมื่อวิเคราะห์พหุตัวแปรพบปัจจัยเสี่ยง ได้แก่ ระดับความรุนแรงของอาการทางจิตที่มากกว่าทุก 10 คะแนน (OR 2.60, 95% CI 1.87-3.63) อายุที่น้อยกว่าทุก 10 ปี (OR 1.69, 95% CI 1.12-2.56) การมีอาชีพ (OR 5.11, 95% CI 2.11-13.26) และการไม่ได้รับยาต้านโรคจิตกลุ่มใหม่ (ไม่รวม clozapine) (OR 4.00, 95%CI 1.54-12.50)

สรุป: ผู้ป่วยจิตเภทประมาณหนึ่งในสามพบภาวะติดนิโคตินร่วมด้วย ซึ่งสัมพันธ์กับระดับความรุนแรงของอาการทางจิต อายุ การมีอาชีพ และการไม่ได้รับยาต้านโรคจิตกลุ่มใหม่ (ไม่รวม clozapine)
