Abstract

บทคัดย่อ

การศึกษาวิจัยครั้งนี้มีวัตถุประสงค์เพื่อพัฒนาระบบการดูแลผู้ป่วยโรคหลอดเลือดหัวใจโดยประยุกต์รูป แบบการจัดการรายกรณี ในสถานการณ์ของโรงพยาบาลสกลนคร การศึกษาครั้งนี้เป็นการวิจัยทางคลินิก แบบกรณีตัวอย่างที่มีการศึกษาวิจัยแบบย้อนหลังและเดินหน้า ดำเนินการระหว่าง พย.49-มีค.50 โดย แบ่งออกเป็น 4 ระยะ คือ ระยะที่ 1 ศึกษาปัญหาโดยกระบวนการวิจัยแบบย้อนหลัง จากการทบทวน เวชระเบียนจำนวน 100 รายของผู้ป่วยกล้ามเนื้อหัวใจตายเฉียบพลันแบบไม่มีภาวะแทรกซ้อน (Uncomplicated Acute ST elevation myocardial infarction; ASTEMI) ระยะที่ 2 พัฒนาแนวทางการปฎิบัติและเครื่องมือวัดผลลัพธ์การดูแลตามกรอบแนวปฏิบัติของการจัดการรายกรณี ระยะที่ 3 การดำเนินงานใน สถานการณ์ตามแนวทางที่พัฒนาระบบ จากระยะวิกฤตในห้องฉุกเฉิน และห้องผู้ป่วยหนัก สู่การดูแลที่หอผู้ป่วยอายุรกรรมในระยะพักฟื้น และการติดตามเยี่ยมบ้าน ระยะที่ 4 การประเมินผลลัพธ์ ผลการศึกษา พบว่าผู้ป่วย Uncomplicated ASTEMI จำนวน 69 ราย ได้รับการรักษาตามแนวทางปฏิบัติ ได้รับการรักษา ด้วย Streptokinase จำนวน 54 ราย โดยมีระยะเวลา door to needle time โดยเฉลี่ย 27 นาที จาก เดิมมากกว่า 120 นาที อัตราการเกิดภาวะแทรกซ้อนจากโรคภายหลังรับไว้ดูแล ได้แก่ CHF, Cardiac arrhythmias, Cardiogenic shock, Stroke, และ Bleeding ลดลงเป็นร้อยละ 2.98, 4.34, 0, 0, และ 8.69 ตามลำดับ อัตราการกลับมารักษาซํ้า ลดลงจากร้อยละ 25.79 เหลือเป็นร้อยละ 1.45 ระยะเวลาวันนอน โรงพยาบาลลดลงจากเดิม 12.5วันเหลือ 4.35วัน ค่ารักษาพยาบาลเฉลี่ยต่อรายลดลงจาก 34,792 บาท เหลือ 21,248 บาท และความพึงพอใจของผู้ป่วยและญาติ อยู่ในระดับดี ร้อยละ 98.13 สรุปจากการ ศึกษาว่ากรณีศึกษาของการพัฒนาระบบการดูแลผู้ป่วยโรคหลอดเลือดหัวใจโคโรนารี่โดยมีผู้จัดการรายกรณี สามารถแสดงให้เห็นการเพิ่มประสิทธิภาพการดูแลผู้ป่วยได้

Abstract

The aim of this study was to develop a caring system for patients with uncomplicated Acute ST Elevation Myocardial Infarction (ASTEMI) based on case management model in the context of Sakolnakorn hospital. This study was a clinical case study research applying retrospective and prospective method of data collection. This study conducted during November 2006-March 2007 had 4 stages: phase 1-exploring problems of caring this target population by reviewing 100 clinical charts of uncomplicated ASTEM; phase 2-developing Clinical Practice Guideline (CPG) and outcome measurements based on case management guideline; phase 3- implementing the developed system (caring from critical period at emergency room and intensive care unit to recovery period at a regular medical floor and home care); phase 4-outcome evaluation. The findings were found that 69 patients with uncomplicated ASTEMI were taken care by the CPG and 54 cases of that received Streptokinase treatment with an average of door to needle time = 27 minutes. After the treatment, the incidences of congestive heart failure, cardiac arrhythmias, cardiogenic shock, stroke, and bleeding were 2.98, 4.34, 0, 0, ·≈– 8.69%, respectively. Percentage of readmission was decreased from 25.79 to 1.45%. A mean length of stay in hospital was reduced from 12.5 to 4.35 days with a mean hospitalized cost reduction from 34,792 to 21,248 baths. Most patients and their family members (98.13%) were satisfied in a good level. In conclusion, the developed caring system based on case management model for patients with uncomplicated ASTEMI showed its effectiveness in term of patients and health care systemûs outcome.
