Self-care knowledge of heart failure patients and follow-up on heart failure treatment using case management

Abstract

This article aims to study self-care knowledge and understanding of heart failure patients, adherence and follow-up in heart failure clinics using case management. This article is a retrospective descriptive study. Data collection was done from 6 case managers and 30 heart failure patients, at Heart Failure Clinic Queen Sirikit Heart Hospital, Faculty of Medicine Khonkaen University. The research instrument were five parts questionnaire include: the personal record of nurses by cases managers, patient Information form Recording of hospital re-entry rates, general health quality assessment model, the self-care knowledge and understanding of heart failure patients. Data analysis used descriptive statistics, frequency, percentage, mean and standard deviation. The results of the study showed that the posttest patients had higher self-care knowledge at the level of 25.50, significantly at the 0.05 level.
