Abstract

การเบ่งคลอดตามธรรมชาติ ได้นำมาใช้ในทางปฏิบัติ แต่มีข้อกังวลในความปลอดภัยต่อผู้คลอดและทารกในครรภ์    วัตถุประสงค์ของบทความนี้เพื่อ ทบทวนผลของการเริ่มเบ่งคลอดครั้งแรกและวิธีการเบ่งคลอดต่อผลลัพธ์ของการคลอด   หลักฐานเชิงประจักษ์ชี้ว่า การชะลอการเบ่งคลอด  ช่วยส่งเสริมการคลอดปกติ ลดการใช้สูติศาสตร์หัตถการได้ มีความปลอดภัยต่อผู้คลอด ทารกในครรภ์และทารกแรกเกิด  แม้ใช้เวลาในระยะที่สองนานกว่ากลุ่มที่เบ่งคลอดทันทีแต่ไม่พบการคลอดยาวนาน  ส่วนวิธีเบ่งคลอดแบบชี้นำช่วยย่นเวลาในระยะที่สองได้ แต่มีผลกระทบต่อการกลั้นปัสสาวะในระยะหลังคลอด  วิธีเบ่งคลอดตามธรรมชาติมีความปลอดภัยต่อผู้คลอด และส่งเสริมสุขภาพทารกแรกเกิด คะแนนแอพการ์ในนาทีที่ 1 และ 5 สูงกว่าทารกในกลุ่มที่เบ่งคลอดแบบชี้นำ    ผู้คลอดควรได้ข้อมูลเกี่ยวกับประโยชน์ ความเสี่ยง และทางเลือกในการเบ่งคลอด พยาบาลควรประเมิน เฝ้าระวังความเสี่ยง และสนับสนุนให้เบ่งคลอดตามความเหมาะสมกับสุขภาวะของแต่ละบุคคล

Spontaneous pushing is currently introduced in delivery units; but maternal and infant safety is mostly concerned to implement this procedure. The objective of this article is to review the effects of delayed pushing and spontaneous pushing on maternal and fetal outcomes. Sufficient evidence for timing and technique of maternal pushing support the practice of delayed pushing and spontaneous pushing.  Delayed pushing can promote vaginal delivery, decrease rate of operative vaginal delivery and be safe for maternal and infant health. The duration of 2nd stage of labor in delayed pushing group is longer than early pushing group; but rate of prolonged 2nd stage of labor is not significance.   Directed pushing can be effective in shortening 2nd stage of labor but may impact on urodynamic functioning.  Spontaneous pushing can improve fetal health as higher APGAR score at 1, 5 minute. Women should be informed of benefit and risk of each technique then practice on their choices.  Each women should be individually assessed and monitored of risk and supported to practice early pushing and spontaneous pushing
