ปัจจัยที่มีอิทธิพลต่อความตั้งใจเลือกการผ่าตัดคลอดของสตรีตั้งครรภ์แรก/Factors Influencing Elective Cesarean Section Intention

บทคัดย่อ

                    การผ่าตัดคลอดโดยไม่มีข้อบ่งชี้ทางการแพทย์มีแนวโน้มเพิ่มมากขึ้น สาเหตุหนึ่งเนื่องจากความตั้งใจเลือกการผ่าตัดคลอด การวิจัยแบบเชิงพยากรณ์ครั้งนี้ มีวัตถุประสงค์เพื่อศึกษาปัจจัยที่มีอิทธิพลต่อความตั้งใจเลือกการผ่าตัดคลอดของสตรีตั้งครรภ์แรก ได้แก่ ทัศนคติต่อการผ่าตัดคลอด การคล้อยตามกลุ่มอ้างอิงในการเลือกผ่าตัดคลอด และการรับรู้ความสามารถในการคลอดของตนเอง ในสตรีครรภ์แรกที่มารับบริการฝากครรภ์ ณ โรงพยาบาลนพรัตนราชธานี จำนวน 120 คน เก็บรวบรวมข้อมูลระหว่างเดือน เมษายน-มิถุนายน พ.ศ. 2559 เครื่องมือที่ใช้ในการวิจัย ประกอบด้วย แบบบันทึกข้อมูลทั่วไป แบบสอบถามทัศนคติต่อการผ่าตัดคลอด แบบสอบถามการคล้อยตามกลุ่มอ้างอิงในการเลือกผ่าตัดคลอด แบบสอบถามการรับรู้ความสามารถในการคลอดของตนเอง และแบบสอบถามความตั้งใจเลือกการผ่าตัดคลอด มีค่าความเชื่อมั่นแอลฟ่าของครอนบาคเท่ากับ .72, .70, .95 และ .87 ตามลำดับ วิเคราะห์ข้อมูลโดยใช้สถิติพรรณนา และวิธีถดถอยพหุคูณ แบบมาตรฐาน

                    ผลการวิจัย พบว่า ทัศนคติต่อการผ่าตัดคลอด การคล้อยตามกลุ่มอ้างอิงในการเลือกผ่าตัดคลอด และการรับรู้ความสามารถในการคลอดของตนเอง สามารถร่วมทำนายความตั้งใจเลือกการผ่าตัดคลอดของสตรีตั้งครรภ์แรกได้ร้อยละ 18.7 (R²= .187, F_3,116 = 8.892) และการคล้อยตามกลุ่มอ้างอิงในการเลือกผ่าตัดคลอดเป็นปัจจัยเดียวที่สามารถทำนายความตั้งใจเลือกการผ่าตัดคลอดของสตรีตั้งแรกได้อย่างมีนัยสำคัญทางสถิติ (β = .364, p < .001)

                    ผลการวิจัยในครั้งนี้ ชี้ให้เห็นถึงความสำคัญของกลุ่มบุคคลอ้างอิงที่มีบทบาทสำคัญยิ่งต่อความตั้งใจเลือกการผ่าตัดคลอดของสตรีตั้งครรภ์ ดังนั้น พยาบาลแผนกฝากครรภ์ควรหากลยุทธ์ในการส่งเสริม ความเชื่อ ค่านิยม ของสตรีตั้งครรภ์แรก ครอบครัว รวมทั้งบุคลากรด้านสุขภาพ ให้เข้าใจ และตระหนักถึงประโยชน์ของวิธีการคลอดเอง เพื่อให้สตรีตั้งครรภ์มีความตั้งใจเลือกวิธีการคลอดทางช่องคลอด มากกว่าการผ่าตัดคลอด

Abstract

                    One reason of inclination of elective cesarean section without medical indications is elective cesarean section intention. This predictive research aimed to examine factors influencing of elective cesarean section intention among primigravida women inclued attitude towards cesarean section, subjective norms of choosing cesarean section and childbirth self-efficacy of primigravidarum receiving antenatal care at Nopparatrajathanee hospital. The sample of 120 primigravidarum. Data were collected from April to June 2016. Research instruments included a demographic record form, the attitude towards cesarean section questionnaire, the subjective norms of choosing cesarean section questionnaire, the childbirth self-efficacy questionnaire and the intention of selecting cesarean section questionnaire. Their Chronbach’s alpha coefficient were .72, .70, .95 and .87 respectively. Data was analyzed by using descriptive statistics and standard multiple regression analysis.

                    Results revealed that attitude towards cesarean section, subjective norms of choosing cesarean section and childbirth self-efficacy join to factor predict elective cesarean section intention among primigravida women, explaining 18.7% of variance (R²= .187, F_3,116 = 8.892). Subjective norms of choosing cesarean section was the only significant predict for elective cesarean section intention among primigravida women. (β = .364, p < .001)

                    The results indicated the important role of group-reference of pregnant women which is the most influential factor for elective cesarean section intention among primigravida women. Therefore, nurses an antenatal care should find strategy to promote primigravida women’s, family and personal group-health have believe and values to realize their values in normal labor for vaginal delivery intention more than elective cesarean section intention.
