Abstract

บทคัดย่อ

            ภาวะไตบาดเจ็บเฉียบพลันเป็นสาเหตุหลักของอัตราการเสียชีวิตในผู้ป่วยระยะวิกฤต ดังนั้นการศึกษาองค์ความรู้ในการดูแลภาวะไตบาดเจ็บเฉียบพลันจึงมีความสำคัญอย่างยิ่ง  การทบทวนวรรณกรรมครั้งนี้ใช้รูปแบบการใช้หลักฐานเชิงประจักษ์ทางการพยาบาลของซูคัพ ในปี ค.ศ. 2000 เป็นกรอบแนวคิดใน การศึกษา ซึ่งประกอบด้วย 4 ระยะคือ 1) การค้นหาปัญหาทางคลินิก 2)  เป็นการสืบค้นหลักฐานเชิงประจักษ์ 3) การนำแนวปฏิบัติไปทดลองใช้ 4) การนำแนวปฏิบัติที่ปรับปรุงไปใช้จริงกับหน่วยงาน บทความดำเนินการในระยะที่ 1 และ 2 ได้งานวิจัย 12 เรื่อง ตามกระบวนการศึกษาประกอบด้วย งานวิจัยระดับ 2 จำนวน 2 เรื่อง งานวิจัยระดับ 3 จำนวน 2 เรื่อง, ระดับ 4 จำนวน 3 เรื่อง, ระดับ 6 จำนวน 1 เรื่อง,ระดับ 7 จำนวน 4 เรื่อง นำงานวิจัยทั้งหมดมาประเมินคุณภาพวิเคราะห์และสกัดสาระสำคัญ สรุปประเด็นจากงานวิจัยได้ 3 ประเด็น คือ 1) การประเมินปัจจัยเสี่ยงที่ทำให้เกิดภาวะไตบาดเจ็บเฉียบพลัน  2) การเฝ้าระวังการดำเนินภาวะไตบาดเจ็บเฉียบพลัน และ 3) การประเมินความพอเพียงของการได้รับสารน้ำในร่างกาย ผลการทบทวนวรรณกรรมครั้งนี้ให้ข้อเสนอแนะวิธีปฏิบัติที่ดีทำให้ได้แนวทางในการประเมินและเฝ้าระวังภาวะไตบาดเจ็บเฉียบพลัน ในผู้ป่วยระยะวิกฤต ที่สามารถนำไปสู่การดูแลและนำไปใช้ในคลินิกหรือพัฒนาเป็นแนวปฏิบัติในการดูแลผู้ป่วยวิกฤตได้ในระยะถัดไป

 

Abstract

            Acute kidney injury is a major cause of mortality in critically ill patients. This evidence based review aimed to summarize clinical knowledge for acute kidney injury assessment among critical patient. A conceptual framework of the evidence based practice model developed by Soukup (2000) was used in this study. It had four phases which consisted of 1) Evidence triggered phase, 2) Evidence supported phase, 3) Evidence observed phase, and 4) Evidence based phase. In this study, phase 1-2 was conducted. A total of 12 studies, there were  2 randomized controlled trials (Evidence level 2), 2 Quasi-experimental studies (Evidence level 3), 3 case controlled studies (Evidence level 4), 1 descriptive study (Evidence level 6), and 4 case studies (Evidence level 7). Result of syntheses EBPs included: assessment of risk factors for acute kidney injury ; acute kidney injury recognition and monitoring tool;  fluid assessment tool; assess and reduce the risk factors for acute kidney injury. The findings of this review provide recommendations of good practices in acute kidney injury recognition and monitoring in critical patients. Further evidence should also be gathered on a regular basis to continue developing the CNPG to ensure quality.
