การพัฒนาบริการเยี่ยมบ้านแบบบูรณาการ 3 ภาคี (สุขภาพ สังคม และชุมชน) ในบริบทของโรงพยาบาลสมเด็จพระยุพราชปัว จังหวัดน่าน

Abstract

     The purposes of this study were to develop innovative integrated home health care among 3 sectors for dependent older persons in community at  Sila- Laeng  health promoting hospital in the context  Pua crown prince  hospital Nan province  Participants were health sectors from health promoting hospital and a secondary care hospital , social sectors from Sila Laeng Municipality Office. Community sectors from head of elderly club, health volunteer, monk and other volunteer. Data were obtained through qualitative data used Long term care need questionnaires among 3 sectors, and qualitative data were obtained through participatory observation, in-depth interviews, focus group discussion, and document analysis, Collecting data used Quantitative data were analyzed using descriptive statistics. Qualitative data were analyzed by using content analysis. Results in 3 phased were: 1) Situation analysis, it was found that older persons had limitations from old ages and health problems. They felt that they were burden to the family and had to rely on family^’s member. They need help from among three sectors; 2) Development phase, the local research team was built awareness was built integrated center, workshop for planning to working together among three parties as well as Dialogue , providing  integrated home health care service; 3) Evaluation, the developed innovations had benefits for older persons and for community. Older persons could get access to integrated health and social service.
