Development of a Community Participatory Pharmaceutical Care Model for Diabetic Patients in Buengkan Province
The aim of this research and development (R&D) was to develop a community–based pharmaceutical care model for diabetic patients in Mueang Bueng Kan district of Bung Kan province. Research was divided into 3 phases. Phase 1 was to diagnose the problem and evaluate the need of pharmaceutical care by community, as well as to study the cases of 86 diabetic patients and relatives. Phase 2 consisted in the development of a model for a representative group of 15 diabetic patients and relatives, 15 public health volunteers and 15 village health volunteers, and then to apply its trial advance to the 86 diabetic participants. Phase 3 was the execution of the model, then to collect data by questionnaire and evaluation of the way to solve the problems of medicine in diabetic patients, in cooperation with the public health personnel in community and volunteers. Study took place from the 8th of February to the 15th of June 2017. Data were analyzed by percentage, mean, standard deviation. Pearson Chi-square correlation coefficient and the statistic Paired t-test were applied for data quality analysis. Results revealed as follows.1. Diabetic patients forgot to take medicine and did not take medicine as indicated on medicine label.2. The community participatory pharmaceutical care model for diabetic patients was successful for problem diagnosis, planning of pharmaceutical care, operation of pharmaceutical care as well as summary and evaluation. 3. After the plan of activities was pretested, it was found that the sample had significantly higher levels of knowledge and appropriate behaviors of using diabetes medicines, including lower blood sugar level. Thus health organizations in provincial areas should apply this community participatory pharmaceutical care model with their diabetic patients continuously.
