คุณภาพชีวิตของผู้สูงอายุไทยที่มีอาการปวดศีรษะที่มีสาเหตุมาจากกระดูกสันหลังส่วนคอ

บทคัดย่อ
การศึกษาที่ผ่านมาพบว่าอาการปวดศีรษะที่มีสาเหตุมาจากกระดูกสันหลังส่วนคอ (cervicogenic headache, Ceh) มีความสัมพันธ์กับการเสื่อมของกระดูกสันหลังส่วนคอและส่งผลกระทบต่อคุณภาพชีวิต ขณะที่ประเทศไทยกำลังเข้าสู่สังคมผู้สูงอายุ ยังไม่มีการศึกษาใดที่ศึกษาถึงผลกระทบของการปวดศีรษะ Ceh ต่อคุณภาพชีวิตของผู้สูงอายุไทย การศึกษานี้มีวัตถุประสงค์เพื่อศึกษาคุณภาพชีวิตของผู้สูงอายุไทยที่มีอาการปวดศีรษะ Ceh และความสัมพันธ์ระหว่างคะแนนคุณภาพชีวิตกับตัวแปรของอาการปวดศีรษะ (ความถี่ ความรุนแรงและระยะเวลาตั้งแต่เริ่มมีอาการปวดศีรษะ) และคะแนนดัชนีความบกพร่องความสามารถของคอ อาสาสมัครเป็นกลุ่มผู้สูงอายุไทยที่มีอาการปวดศีรษะ Ceh จำนวน 25 คน (อายุเฉลี่ย 64.88 ± 3.55 ปี) และกลุ่มควบคุมที่ไม่มีอาการปวดศีรษะ จำนวน 25 คน (อายุเฉลี่ย 64.60 ± 3.52 ปี) อาสาสมัครทั้ง 2 กลุ่มตอบแบบสอบถาม SF-36 กลุ่มที่มีอาการปวดศีรษะ Ceh ตอบแบบสอบถามอาการปวดศีรษะและแบบประเมินดัชนีความบกพร่องความสามารถของคอฉบับภาษาไทยร่วมด้วย ผลการศึกษาในภาพรวมพบว่ากลุ่มที่มีอาการปวดศีรษะ Ceh มีคุณภาพชีวิตทั้งทางด้านสุขภาพกายและสุขภาพใจลดลงอย่างมีนัยสำคัญทางสถิติเมื่อเปรียบเทียบกับผู้สูงอายุในกลุ่มควบคุม (p < 0.05) คะแนนคุณภาพชีวิติในมิติด้านสุขภาพกายมีความสัมพันธ์เชิงลบอย่างมีนัยสำคัญกับคะแนนดัชนีวัดความบกพร่องความสามารถของคอ (ค่า r = - 0.57, p < 0.05) ผลการศึกษาครั้งนี้แสดงให้เห็นว่าอาการปวดศีรษะ Ceh มีผลกระทบต่อคุณภาพชีวิตทั้งทางสุขภาพกายและสุขภาพใจของผู้สูงอายุไทย การให้โปรแกรมดูแลรักษาที่เหมาะสมจำเป็นสำหรับการเพิ่มคุณภาพชีวิตของผู้สูงอายุไทยที่มีอาการปวดศีรษะ Ceh
