มุมงอข้อสะโพกที่เหมาะสมสำหรับการทดสอบความแข็งแรงด้วยมือของกล้ามเนื้อเหยียดสะโพกในท่านอนหงาย

บทคัดย่อ

บทนำ การทดสอบความแข็งแรงของกล้ามเนื้อเหยียดสะโพก (hip extensor: HE) ด้วยมือ (manual muscle test: MMT) ในท่าที่นิยมใช้ทางคลินิก (traditional: Trad)  มีท่าเริ่มต้นของการประเมินเกรด 3 อยู่ในท่านอนคว่ำ ทำให้มีข้อจำกัดในการตรวจผู้ป่วยที่ไม่สามารถนอนคว่ำได้  การตรวจความแข็งแรงของ HE ในท่านอนหงายจึงเป็นทางเลือกที่น่าสนใจ  มีผู้ศึกษาความเชื่อถือได้ และความเที่ยงตรงของการตรวจประเมิน MMT ของ HE ในท่านอนหงาย ซึ่งเริ่มด้วยการงอข้อสะโพกของขาข้างที่ทดสอบให้พ้นพื้น  ก่อนให้ผู้ป่วยพยายามเหยียดข้อสะโพกและยกสะโพกขึ้น ให้ขาและลำตัวอยู่ในแนวเดียวกันให้มากที่สุด  อย่างไรก็ตาม ยังไม่มีการศึกษาถึงความสอดคล้องกันของการให้เกรดกล้ามเนื้อ HE ระหว่างการทดสอบ MMT ในท่านอนหงายกับท่า Trad ในอาสาสมัครคนเดียวกัน  รวมทั้งยังไม่มีผู้ทำการศึกษาเพื่อประเมินมุมงอข้อสะโพกที่เหมาะสมสำหรับการทดสอบ HE ในท่านอนหงาย  วัตถุประสงค์ เพื่อประเมินมุมงอข้อสะโพกที่เหมาะสมสำหรับการทดสอบ MMT ของกล้ามเนื้อ HE ในท่านอนหงาย โดยเปรียบเทียบเกรดของกล้ามเนื้อเมื่อประเมินที่มุมงอข้อสะโพก 30, 45 และ 60 องศา กับเกรดที่ได้จากท่า Trad ในอาสาสมัครคนเดียวกัน  วิธีการ  การศึกษานี้ทำในอาสาสมัครที่เป็นผู้ป่วย 17 คน และผู้ที่มีสุขภาพดี 18 คน รวมกล้ามเนื้อ HE ที่ถูกประเมิน 51 ข้าง อาสาสมัครที่เข้าร่วมการศึกษาต้องสามารถนอนคว่ำได้และมีเกรดของ HE เมื่อประเมินในท่า Trad อยู่ระหว่าง 2 ถึง 5  อาสาสมัครได้รับการทดสอบ MMT ทั้งในท่า Trad และท่านอนหงายที่มุมงอข้อสะโพก 30, 45 และ 60 องศา  ลำดับของการประเมินแต่ละมุมเป็นแบบสุ่ม  วิเคราะห์ความสอดคล้องกัน (agreement) ของเกรดกล้ามเนื้อที่ได้จากการทดสอบในท่านอนหงายที่มุมงอข้อสะโพก 30, 45 และ 60 องศา กับในท่า Trad โดยใช้ Quadratic weighted Kappa  ผลการศึกษา พบว่าค่า agreement ระหว่างเกรด MMT ของกล้ามเนื้อ HE ในท่านอนหงายมุมงอข้อสะโพก 30, 45 และ 60 องศา กับท่า Trad มีความสอดคล้องกันอยู่ในระดับปานกลาง คือ 0.54, 0.60 และ 0.42 ตามลำดับ โดยที่มุมงอข้อสะโพก 45 องศา มีค่าความสอดคล้องกับเกรดกล้ามเนื้อในท่า Trad มากที่สุด    การนำไปใช้ การศึกษานี้สามารถนำไปใช้ประเมิน MMT ของกล้ามเนื้อ HE ในผู้ป่วยที่ไม่สามารถนอนคว่ำ โดยทำการประเมินในท่านอนหงายและงอข้อสะโพก 45 องศา
