Abstract

การวิจัยและพัฒนานี้ (Research and Development) มีวัตถุประสงค์เพื่อพัฒนารูปแบบการดูแลเท้าของผู้ป่วยโรคเบาหวานด้วยรางไม้ไผ่ขั้นตอนการวิจัยแบ่งออกเป็น 4 ระยะประกอบด้วยระยะที่ 1 ประเมินสถานการณ์โรคระยะที่ 2 สืบค้นและคัดสรรภูมิปัญญาท้องถิ่นระยะที่ 3 ทดลองใช้ภูมิปัญญาท้องถิ่นที่ได้รับคัดสรรและระยะที่ 4 การประเมินผล


       ประเมินสถานการณ์เป็นการศึกษาภาวะแทรกซ้อนโรคเบาหวานที่มารับบริการโรงพยาบาลส่งเสริม สุขภาพตำบล จำนวน 7 แห่งพบว่า ภาวะแทรกซ้อนโรคเบาหวาน 3 อันดับแรก คือ ภาวะไตวาย อาการชาที่เท้า ต้อกระจกและเมื่อค้นหาปัจจัยการเกิดภาวะแทรกซ้อนของโรคเบาหวานในผู้สูงอายุการตัดสินใจเลือกภาวะแทรกซ้อนที่ต้องการแก้ไขและแนวทางการดูแลโดยวิธีการสนทนากลุ่มกับตัวแทนที่เกี่ยวข้องกับการดูแลสุขภาพผู้สูงอายุโรคเบาหวาน คัดเลือกกลุ่มตัวอย่างแบบเจาะจง จำนวน 20 คนพบว่า กลุ่มตัวอย่างต้องการค้นหากิจกรรมป้องกันโรคแทรกซ้อนจากอาการชาปลายประสาทที่เท้าโดยใช้ภูมิปัญญาท้องถิ่นและมี  ความเสี่ยงการตีบของหลอดเลือดส่วนปลายมากกว่าร้อยละ 28 ผู้วิจัยได้นำการใช้รางไม้ไผ่ในการนวดเท้ามาพัฒนาวิธีการดูแลเท้าของผู้ป่วยโรคเบาหวานวิเคราะห์โดยผู้เชี่ยวชาญโดยกำหนดบทบาทหน้าที่ความรับผิดชอบการดำเนินงาน และพัฒนาทักษะการดูแลเท้า ดำเนินกิจกรรมการใช้รางไม้ไผ่ในการนวดเท้าร่วมกับการสอนสุขศึกษา ตามแนวคิดการรับรู้สมรรถนะแห่งตนกลุ่มตัวอย่างคัดเลือกแบบเจาะจงจำนวน 50 คน โดยแบ่งเป็นกลุ่มทดลองและควบคุมกลุ่มละเท่าๆ กัน


            ผลการศึกษาพบว่าหลังการดำเนินกิจกรรม กลุ่มทดลองมีค่าเฉลี่ยจำนวนจุดชาที่เท้าโดยโมโนฟิลาเมนและการตีบของเส้นเลือดส่วนปลายน้อยกว่าก่อนการทดลองและน้อยกว่ากลุ่มควบคุมอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 ระยะประเมินผลหลังการดำเนินกิจกรรมเป็นระยะเวลา 12 สัปดาห์พบว่าระดับความพึงพอใจของผู้ป่วยโรคเบาหวานที่มีต่อการนวดเท้าด้วยรางไม้ โดยรวมอยู่ในระดับพึงพอใจมาก (gif.latex?\bar{x}= 4.47, SD=.70)
