บทคัดย่อ

          การวิจัยเชิงคุณภาพด้วยวิธีการศึกษาเชิงชาติพันธุ์วรรณาครั้งนี้ มีวัตถุประสงค์เพื่อเข้าใจและอธิบายการดูแลผู้ป่วยระยะสุดท้ายแบบประคับประคองของชุมชนในสังคมวัฒนธรรมอีสาน ระยะเวลาในพื้นที่ 2 ปี ตั้งแต่เดือน เมษายน 2553 ถึง พฤษภาคม 2555 ผู้ให้ข้อมูลหลักประกอบด้วย ผู้ป่วยโรคเรื้อรังในชุมชนที่ต้องการการดูแลที่ซับซ้อน ได้แก่ ผู้ป่วยโรคมะเร็ง ผู้ป่วยไตวายระยะสุดท้าย และผู้ป่วยอัมพาต จำนวน 10 คน ผู้ให้ข้อมูลรอง ได้แก่ สมาชิกในครอบครัวที่ดูแลผู้ป่วยโรคเรื้อรังที่อยู่ในระยะสุดท้ายของชีวิต 10 คน เลือกเจาะจงตามเกณฑ์ เก็บข้อมูลที่ตำบลแห่งหนึ่งใน อ.วารินชำราบ

จ.อุบลราชธานี เก็บข้อมูลจากการสัมภาษณ์เชิงลึก การสนทนากลุ่ม การสังเกต และการบันทึกภาคสนาม วิเคราะห์ข้อมูลด้วยการวิเคราะห์เนื้อหา

          ข้อค้นพบการดูแลผู้ป่วยระยะสุดท้ายแบบประคับประคองของชุมชน เป็นแบบที่ภาษาอีสานเรียกว่า “เบิ่งแยง” มีความหมายถึงการ “อยู่ก็สบาย ไปก็สงบ”  เป็นการดูแลที่ต่อเนื่องและเป็นแบบแผน 4 ลักษณะ เริ่มจากการออกจากโรงพยาบาลมาอยู่ที่บ้านซึ่งเป็นจุดเริ่มต้นของการดูแลในชุมชน เรียกว่า ตุ้มโฮม หมายถึง ญาติและคนในชุมชนมีการบอกกล่าวและชักชวนกันมาร่วมให้กำลังใจ ซึ่งสะท้อนว่าผู้ป่วยและสมาชิกในครอบครัวมีความสำคัญต่อชุมชน  ระยะต่อมาเป็นการปฏิบัติกับของรักษาของตน เป็นระยะที่ผู้ป่วยกราบไหว้บูชาสิ่งที่ตนนับถือ ในขณะที่ครอบครัวและชุมชนหันหน้าเข้าหากันเพื่อปรึกษาหาทางเลือกการดูแลทางจิตใจและจิต-วิญญาณควบคู่ไปกับการดูแลความไม่สุขสบายทางกาย มุ่งการทำความดีงาม และลดความบาดหมางในครอบครัว เมื่ออาการของผู้ป่วยไม่ดีขึ้น ผู้ป่วยและญาติต่างยอมรับว่าหมดหนทางรักษาแล้ว เป็นระยะของการซอมใจ สะท้อนการเตรียมการตายและหมดห่วง คนในชุมชนยิ่งมีความเอาใจใส่ มีการหมุนเวียนเยี่ยมเยียนถามไถ่ เมื่อใกล้ถึงระยะสุดท้ายเป็นการตายงาม   ซึ่งกระบวนการทั้ง 3 ระยะนี้ ครอบครัวและชุมชนในบริบทอีสานมักร่วมกันตัดสินใจ และทำตามประเพณีวัฒนธรรม แต่ไม่สามารถจัดการด้านอาการไม่สุขสบายและทุกข์ทรมานได้ทั้งหมด เนื่องจากเงื่อนไขของการดูแล ได้แก่ ศักยภาพการจัดการโดยครอบครัว และแนวทางการดูแลของชุมชน รวมทั้งระบบบริการสุขภาพ แนวทางและมาตรฐานการดูแล และคุณภาพของการดูแล ผลการวิจัยครั้งนี้ ช่วยให้บุคลากรสุขภาพมีมุมมองใหม่ในการเข้าใจครอบครัวอีสานอย่างลึกซึ้ง และเห็นการดูแลผู้ป่วยระยะสุดท้ายของครอบครัว การพยาบาลจึงควรออกแบบการดูแลที่มีความละเอียดอ่อนทางวัฒนธรรม

"Yoo koh sabuy-Pai koh sa-ngob": The Palliative Care for End-of-Life Patients in Northeast Socio-culture

Abstract

The majority of chronic patients in northeast Thailand (Isaan) spends their end of life at home, but little is known about how those patients live their lives in the community. The purpose of this ethnographic study was to describe the palliative care for end-of-life patients in a sub-district area of Thailand between April 2010 and May 2012. Ten patients with chronic diseases, including cancer, end stage renal failure, and paralysis, participated in the study. Ten caregivers, 3 folk wisdom and ritual practitioners, 15 villagers, 13 health volunteers and 2 health workers in primary care units were general informants. In-depth interviews, group discussions, observations and field notes were used to collect data. Data were analyzed by using content analysis.

The findings show that Isaan (northeastern) families continually take care of end-of-life family members by using the theme of  "Yoo koh sabuy-Pai koh sa-ngob (comfort live and peaceful death)". Four patterns of cultural care were found. Firstly, “Tum home (collecting relative)” is a period when patients return home from the hospital. Their families, relatives, friends, and neighbors would come to bless them. Next, they would go to the stage of “Kan pa tee but kub kong rak sa (practice the presence of holy spirit)” when they were looking for any better physical and spiritual care. The patients and families would compromise and try to reduce conflicts among themselves. The next stage was “Som jai (monitoring)”. This was the time when all parties accepted the truth that the patients were dying. The final stage was “Tai Ngam” (peaceful death) whereby the patient passes away with serenity.  Community members would visit and support the patients and families.  

          This study provides health care professionals new lenses to look deeper into northeast families’ socio-cultural aspects to see how they take care of end-of-life patients. Nursing care plans should be created with cultural sensitivity.
