It is the thinking process which nurses use to assess and interpret information, evaluate crucial cues and make decisions regarding appropriate nursing care. Effective clinical reasoning processes can promote appropriate clinical decision making to improve quality and safety in nursing practice. Therefore, clinical reasoning is important for nursing practice. Teaching and learning for nursing students needs to focus on developing their clinical reasoning ability. Several teaching and learning methods, including think aloud, questioning, concept mapping, analogy and reflective thinking, can be employed to develop nursing students’ clinical reasoning skills. This ability can bring effective clinical decision making to nursing practice.</p> สมจิตต์ สินธุชัย พัชนียา เชียงตา ณัฐวุฒิ บุญสนธิ ##submission.copyrightStatement## https://creativecommons.org/licenses/by-nc-nd/4.0 2018-09-10 2018-09-10 28 2 27 40 Development of Nursing Practice Guidelines for Pain Management in Severe Traumatic Brain Injury and Multiple Injury Patients https://www.tci-thaijo.org/index.php/tnaph/article/view/144721 <p><strong>Abstract</strong></p> <p>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; The development of nursing practice guidelines for pain management in severe traumatic brain injury and multiple injury patients employed the evidence-based practice model of Soukup in 2000 which consisted of 1) pain assessment, 2) pain management, and 3) prevention of risk factors of pain stimulating severity of pathology. The content validity was performed (CVI=.93). The practice guidelines was applied by 29 nurses in an accident and surgery department. The possibility of application, and satisfaction with the application of the guideline were assessed. It was found that 1) the possibility of application was good at 72.9 percent, 2) the satisfaction with the application was good at 86.5 percent, and 3) the satisfaction with the nursing practice regarding pain mangement with medicines and involvement with the practice development was good at 75.86 percent. The practice also gave the nurses confidence to perform their role independently in order to manage pain, which was good at 75.17 percent. The nursing practice followed the pain management practice guidline&nbsp; at 80 percent. The nurses were satisfied with the pain assessment applied with the patients who were not able to communicate, especially those who were unconscious and those whohad pain killer injection (12 patients). The stratified random sampling was employed to select the sample. Instruments used for data collection included 1) Demographic Data, 2) Children’s Depression Inventory-CDI, 3) Resilience Inventory, 4) Life Asset Inventory, 5) Family Relationship Questionnaires, 6) Friendship Intimacy Questionnaire and 7) Personal Resource Questionnaire. The questionnaires part 2-7 yielded Cronbach’s Alpha of .84-.95. Descriptive statistics, Pearson’s product moment correlation coefficient, and stepwise multiple regression analysis were</p> <p>employed for data analyses.&nbsp;</p> <p>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; The study findings revealed that this sample had a mean depression score of 11.27 (SD=8.92), 27.27 percent of this sample had depression (CDI ≥ 15).&nbsp; Stepwise multiple regression analysis demonstrated that life asset, family relationship, and social support together could significantly explained 21.00 % of the variance for depression (R2=.210, F=29.340, p&lt;.001). A purposive sampling technique was used to recruit&nbsp; of 56 patients diagnosed with asthma from an out-patient clinic of a primary care unit in Bangkok. The sample was divided into two groups. The experimental group consisted of 28 asthma patients while the control group consisted of another 28 patients. The experimental group received the program of inhaler application along with knowledge related to perceived self-efficacy and health behavior while the control group received the usual care for asthma.&nbsp; The questionnaire consisted of demographic information, self-efficacy scale or SES (CVI=.8; alpha=.85) for asthma, and health practice scale or HPS (CVI=.9; alpha=.88). The data were obtained during three phases, including before receiving the program,&nbsp; immediately after finishing the program, and 4 weeks after completion of the program. The data were analysed using descriptive statistics, Chi-square, One-way repeated measure ANOVA, and independent-t test.</p> <p>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; &nbsp;&nbsp;The results showed that the average scores of SES and HPS in the experimental group were significantly higher than those of the control group when receiving the program immediately after completion, and 4 weeks after completion (p&lt;.01).&nbsp; The average scores of SES and HPS in the experimental group when receiving the program immediately after completion and 4 weeks after completion, were significantly higher than those of the group before receiving the program (p&lt;.01). The study adopted the conceptual framework on HIV disclosure by women developed by Sowell et al. A purposive sampling technique was used to recruit 70 HIV-infected women who received medical services at the Medicine Outpatient Department of Ramathibodi Hospital during 2016-2017, and who were living together with their partners. The instruments for data collection included a questionnaire on demographic data, a questionnaire on perception of stigma of HIV-infected patients, and a questionnaire on perception of social support for HIV-infected patients. The data were analysed using descriptive statistics and</p> <p>Spearman’s rank correlation coefficient. This study found that the majority of the sample were aged between 30-39 years (44.3%), were educated to a secondary level (31.4%) and lived together with their partners from 6 months to 35 years,</p> <p>with an average period of 9.33 years (SD=8.94). The HIV infection was mainly from heterosexual relationships (85.7%). Most samples disclosed their HIV infection to their partners (70%). Their overall perception of stigma was at a moderate level (67.1%).</p> <p>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; The perception of social support was at a moderate level for 50% of the samples and at a high level for 48%. Based on the Spearman’s correlation analysis, age, education, perception of stigma, and perception of social support were associated with HIV disclosure by Thai women to their partners with a statistical significance (p&lt;0.05). This predictive correlational research aimed to study the correlation and predictive power of risk perception affecting risk perception behavior from using cosmetic products of nursing students. The sample consisted of 190 first year to third year nursing students enrolled for academic year of 2017, and recruited by a stratified and systematic sampling method. The instrument was risk perception and risk preventing behaviors from using cosmetic products questionnaire. The content validity of the questionnaire was confirmed by a panel of three experts, yielding a CVI of .80. The reliability of the questionnaire was tested using Alpha-Cronbach Coefficient, revealing value of .71. Therefore, this study applied Facebook, a popular social media network, in problem-based learning in order to</p> <p>reduce the need for resources for the Nursing Care of Persons with Mental Health Problems course for the third year nursing students at the Boromrajonani College of Nursing, Phayao. The effects of problem-based learning using Facebook (PBLF) on knowledge and self-directed learning readiness were measured. In addition, students’ perceptions on problem-based learning and lecture-based learning were also examined. The results showed that knowledge and self-directed learning readiness increased after the PBLF (p&lt;0.01). The students’ perceptions on problem-based learning were more positive than lecture–based learning.&nbsp; The analysis of students’ opinions regarding PBLF revealed the following views: new and interesting learning styles, lack of confidence on subject contents, time and communication problems. The findings suggest that PBLF has positive effects on learning and therefore it is a useful teaching method when the number of faculty members is insufficient. The conceptual model guiding this study was derived from the Chain of Survival concept and Theory of instructional design. The sample consisted of 30 high-school students in northeastern Thailand, using a one group pretest –</p> <p>posttest design. The instruments used were self-administered questionnaires, pretest and posttest, performance appraisals of CPR skill, and a set of training materials developed by researchers. The study was conducted between August and September 2016.&nbsp;</p> <p>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; The results showed that the knowledge and skill before receiving intervention were significantly different to those of after receiving the intervention at the .05 level. The findings suggest that this experimental program should be expanded to other school students.
