The development model for rational drug use : a case study of type 2 diabetic patients in Namyuen Hospital, Ubon Ratchathani Province

Abstract

This action research aimed to develop a model of pharmaceutical care for rational drugs use in type 2 diabetic patients at Nam Yuen Hospital, Nam Yuen District, Ubon Ratchathani Province. The target group was purposive sampling of 12 health professionals. The instruments used for data collection were created questionnaires and assessment forms. Data were analyzed using descriptive statistics: mean, percentage, standard deviation and inferential statistics such as Wilcoxon Signed Rank Test. Qualitative data were collected by observing and recording and analyses by content analysis.

The research results found that the development process consists of 8 steps: 1) the contextual study, 2) appointed the leading team, 3) collect and analyze data, 4) planning 5) implementation, 6) observation and monitoring, 7) evaluation and 8) the lessons learned. It was found that significantly increasing in knowledge and participation in RDU care in the providers. As a result, the hospital has passed a rational evaluation of the RDU indicators, although there are some indicators that are not yet met, however, we can see positive improving implement on RDU criterion. It was found that the primary new care model called TEAM-FC was developed which was based on the principle "Good information, - Empirical evidence, - Generating the awareness, - Follow-up, - Reflection, - Solve problems with the team".

In summary, the key success factors comprised of the participation and cooperation of health professionals' teams at all levels. This study will be benefited to the system clinical practice guideline combined with the evidence based on local health information including strongly policy support at the provincial and national level.
