ผลการติดตามเย่ยี มบ้านเด็กโรคหืด ในอำเภอท่าบ่อ จังหวัดหนองคาย*

บทคัดย่อ

     การวิจัยนเป็นการวิจัยเชิง ปฏิบัตการ เพื่อติดตามเยี่ยมบ้านเด็กโรคหืดจุดประสงค์เพื่อศึกษาโปรแกรมเยี่ยมบ้านจะมีประสิทธิผลในการเพิ่มความรู้และความสามารถของผู้ดูแลในการจัดการการดูแลเด็กโรคหืดและเพิ่มความจุปอดของเด็กโรคหืด การลดจำนวนครั้งการใช้บริการที่ห้องฉุกเฉิน และการเข้านอนรักษาในโรงพยาบาลจากอาการหืดกำเริบของเด็กโรคหืดหรือไม่ และ พฤติกรรมการดูแลเด็กโรคหืดที่บ้านของผู้ดูแลเป็นอย่างไร กลุ่มตัวอย่างได้แก่เด็กโรคหืดอายุ1-15 ปี และผู้ดูแล ที่เข้ารับการรักษาในคลินิกโรคหืดเด็กโรงพยาบาลสมเด็จพระยุพราชท่าบ่อ มาแล้วอย่างน้อย 3 เดือน ยังมีอาการหืดกำเริบ จำเป็นต้องเข้านอนรักษาในโรงพยาบาลหรือพ่นยาขยายหลอดลมที่ห้องฉุกเฉิน จำนวน 28 คน โดยติดตามเยี่ยมบ้าน 2 ครั้ง ห่างกันประมาณ 1 เดือน และติดตามประเมินผลในคลินิกโรคหืดเด็ก 1ครั้งหลังจากการเยี่ยมบ้านครั้งที่ 2 ประมาณ 3 เดือน เครื่องมือที่ใช้ในการวิจัย ประกอบด้วย คู่มือคำแนะนำการปฏิบัติตัวสำหรับเด็กโรคหืด แบบสอบถามข้อมูลส่วนบุคคลของเด็กและผู้ดูแล แบบสัมภาษณ์การจัดการดูแลเด็กโรคหืดของผู้ดูแล และแบบบันทึกการติดตามเยี่ยมบ้านเด็กโรคหืด วิเคราะห์ข้อมูลเชิงปริมาณและเชิงคุณภาพ
      ผลการวิจัยพบว่า หลังการติดตามเยี่ยมบ้าน คะแนนความรู้เกี่ยวกับการจัดการตนเองของผู้ดูแลหลังติดตามเยี่ยมบ้านสูงกว่าก่อนการติดตามเยี่ยมบ้าน ปัจจัยกระตุ้นที่พบมากที่สุดและเป็นสิ่งกระตุ้นที่ผู้ดูแลไม่ให้ความสำคัญ ได้แก่ ฝุ่นละออง ควันจากท่อไอเสียรถยนต์ และการใช้ยาสูดพ่นที่ไม่ถูกต้อง การประเมินผลลัพธ์ หลังติดตามเยี่ยมบ้าน พบว่า เด็กโรคหืดกลุ่มนี้ ไม่ได้ใช้บริการที่ห้องฉุกเฉินและเข้านอนรักษาในโรงพยาบาลด้วยอาการหืดกำเริบ และยังพบว่าค่าความจุปอดเพิ่มขึ้น มากกว่าร้อยละ 80

The Effect of Home Visits for Children with Asthma in Thabo District Nongkhai Province*

Abstract

     This action research study was used to explore the follow-up of home visits for children with asthma. The purpose of the home visit program was to increase the knowledge and ability of caregivers to manage children with asthma, and to increasing the lung capacity of children with asthma so as to reduce the number of times the emergency room is used and hospitalized needed due to exacerbated symptoms. The samples were chosen using a specific selection method so as to be representative of children with asthma aged 1-15 years, and their caregivers who had received treatment in the pediatric asthma clinic at Thabo Crown Prince Hospital over a period of at least three months. They had severe asthma and needed to admit or required Bronchodilator inhalers at emergency room. Twenty-eight children with asthma and their caregivers were followed by 2 home visits over one month, followed by a home visit after 3 months and after home visit 3 month evaluated at pediatric asthma clinic 1 time. The tools used include a handbook for manual of advice practice for children with asthma, questionnaire of personal information of the children and their caregivers, interviews with the caregivers, and the record of home visits of children with asthma. Data were analyzed by quantitative and qualitative methods.
     The results showed that after home visits, the knowledge scores regarding self-management of the caregivers was higher than before home visits. The most common and unconcern triggers were dust and smoke from car exhaust and improper inhaler use. The evaluation after home visits showed that the subjects did not request admission or Bronchodilator inhalers at the emergency room because of asthma exacerbations. All subjects had the peak flow meter increased by more than 80%.
